While assisting a subcontractors employee experiencing a medical emergency employee felt pain in lower back. Employee tried to manage the pain by stretching and doing at home exercises. Employee requested medical treatment on 11/17/2025.

Showing all 9Employee was descending stairs from the crushing plant control room when EE slipped and hit EE's elbow on the stairs as EE slid down.
Load was dumped into hopper when worker saw a piece of rebar that needed to be pulled out. When he reached to pull the rebar out, a piece of concrete rolled down the wing wall smashing his left thumb between concrete and wall of hopper.
Lying on back on the ground, replacing return roller on under jaw belt.
The laborer was under a moving conveyor; when he reached up the belt caught his hand and pulled his arm between the return roller and conveyor belt pressing his forearm against the frame of the conveyor.
A stick fell over the top of the cone crusher, the operator left the control room to remove the stick and a rock struck the back of his right hand.

Employee opened the hatch door, failing to latch the door open and standing partially through the hatch, it swung closed, hitting his head.
