Lost balance trying to install rock bolt. Employee had a controlled fall to the ground. The jack-leg landed on the employee's right foot, right above the metatarsal boot protection.

Mill operator was atempting to dump a 5 gallon bucket of garbage into a large dumpster, while lifting up to place in the dumpster some of the contents fell out of the bucket and struck the top of the operators hand resulting in a laceration of approx. 1.5 inches in length requiring 3 stitches.

EE was to prevent entry into blasting area. Pickup used to block traffic moved too close to intersection of blasting area & main drift. Fly rock from blasting area traveled 500 ft. & struck employee sitting in pickup. EE was struck by small rock which fractured safety glasses causing glasses to penetrate eye. Evaluation ongoing. EE stated he drove in prior to shot's completion.

Employee was walking on main haul road from administration area to his office on the mill bench. He slipped and fell landing on his left wrist resulting in a fracture of that wrist. Road was snow covered and had gravel showing on the surface.

Employee was carrying down a paste sample( 1 liter sample weighing approx. 1 lb.) down the stairs to check the density for the pour sheet. Employee misjudged the last step and lost his footing and fell to the ground resulting in a fracture of the lower left leg.

While bolting low ribs with a jackleg, the leg kicked out causing the machine to fall. Employee picked up the machine and in the process of repositioning it his left index finger was pinched by the chuck tender. This caused a cut to the left index finger, that required 6 stitches to repair, and employee was given a full medical release back to work.

While performing duties for the on site fire dept. employee was doing hose operations,while hosing he tripped and twisted right knee.

Employee had a sudden cardiac arrest due to natural causes

Mine was riding in a Getman when he put his hand on the outside of the ROPS. The vehicle swayed to the side and his hand was pinched between the rib and the vehicle. several stiches were required. No lost time.

EE was lifting around 20 lbs tray for a salad bar. While lifting, she twisted her lower back, lower back muscle stain. She was distibuted with 2 doses of 30 mg of Toradol on site. Rotational s chedule, RTW date is 8/6/08.

Employee was working fro Teck-Cominco at the time and was beating on a pressurized paste pipe when the plug broke loose. The pipe with the pressure releasing swung over & knocked employee over on his right shoulder, breaking his collar bone.

Employee had his finger get caught in a pinch point in a jack leg that caused an injury that required stitches to close the wound.

While operating was moving liner into place, EE pushed liner to rotate in position. The liner bumped the wall pinching his finger between wall and liner, Resulting in a fractured finger on his left hand

Employee was hanging a hanger for hose when a small rock fell through the screen hitting him on his hand. The sharp rock lacerated his finger resulting in stitches.

While doing a Mine Rescue training senerio employee was climbing up a gob pile to search for missing miner he put his right hand on a bolt coming out of rib at that time a small rock came down from the back behind the wire and struck him on his left hand

Employee was cleaning out paste hopper, when he slipped and fell, causing left leg injury. Was reported as possible pulled muscle. Approx 1 1/2 wks later was found to have a hairline fracture of left small toe. Was originally reported as FA but due to fracture, needed to be changed to MSHA reportable. That is why being reported past deadline.

While closing the red chain valve that directs water to different tanks, the door next to the work area had malfunctioned and tarps had been put in place. Due to the cold weather the floor had become very slick with ice build up. As he was closing the valve he lost footing and fell to the ground resulting in an upper left arm fracture

Employee recived 2 stiches in rt. elbow from a vehicle accident.

Employee was stepping out of a drill when he says he stepped on a loose piece of rock, lost his footing and felt a pop in his knee. The employee did not report the injury at the time of the incident. The employee reported the injury before he left the mine site on Feb 14th.

Jackleg started falling, pushing him off the power pack. He fell and broke his left ankle, requiring surgery.

Employee was stepping down off of a getman powder truck onto a 5 ft. ladder as he continued down the ladder his foot slipped on the second to last foot rung as he reached up with his left hand to grab his balance he cut his left ring finger as it slid down the ladder

employee did not return back for his first scheldued shift back from his days off. When talking to employee he complained of lower back pain from unknown causes while working here at the mine. he believes it might be from driving around in 50 ton haul trucks along with kabota tractors. He has a history of lower back problems

While tightening a bolt on a 8" vic clamp on a paste pipe line, he was pulling on the wrench when he felt a pop in his right shoulder and it gave out and had alot of pain.

While digging at a lifter a rock fell from the face striking the employee's hard hat creating a laceration.

CHT 04 cat 740 haul truck was rounding corner and loss control of truck and drove over the berm

While transferring oil from 55 gal. metal drum to the waste oil tank,the vent was overflowing and employee quickly shut off the valve and by do so caught his right middle finger at the tail end of a double hose clamp resulting in a laceration to that finger

Employee slipped and fell landing on his right hand cutting his thumb on a sharp rock. Employee did not have his gloves on at the time.

While trying to retrieve the loading spoon from inside the boom assembly on GME 02 emulsion truck, his partner who was in the basket at the time accidently touched one of the control levers causing the boom to retract breaking employee's left arm. At the time the engine on the unit was running.

EE was installing ground support in a raise when a piece of loose fell from the footwall striking EE's right foot above his metatarsal, causing a laceration that resulted in nine sutures. EE received full release.

Employee while walking on level ground, slipped and lost footing due to loose rock and muddy conditions. As a result employee twisted his right knee.

Employee was hooking up pump to the boom, took gloves off to pull pin out of clevis. #1 boom was running-reaming last hole. The hole finished, turned to go and pull steel out of hole when he tripped and fell down on broken quartz, cutting his left palm near his thumb.

Employee was moving a portable bandsaw when the base came in contact with the track rails inlayed in the shop floor. Employee tried to force the unit over the rails causing the saw to fall on his leg. Employee received a 3 inch laceration. Employee received 15 stitches.

While pushing material down in a dumpster a piece of wire cable sprung out and contacted the employee's chin causing a small laceration. Five stitches were required.

Employee was pulling on a hose when he felt a sharp pain. Later diagnosed as a broken rib. No blunt injury occurred. Employee is unsure of how it happened.

Putting resin in hole a 3" rock fell through the screen hitting employee on right wrist causing employee to step back. Stepping back employee lost his footing and fell on his left shoulder.

After employee finished fueling D7 Dozer and was stepping down he lost his balance and fell approx. 2-3 ft. to the ground landing on his left wrist and shoulder resulting in a left wrist fracture.

While employee was climbing down ladder he turned to get off and struck his right elbow on the aluminum door frame. Incident was not reported to his supervisor.

while walking in the 1400 stope acess area on flat surface he inadvertantly stepped on a small rock and rolled his right ankle.

Employee was walking on a catwalk in a poorly lit area and stepped on a 3" section of PVC piping that was laying on the catwalk. The pipe rolled and caused the employee's ankle to roll.

Employee was lifting when he lost his footing and felt pain in mid back region.

Employee was drilling an electrical box on the Maclean bolter when the drill penetrated through the box cutting the back of his hand.

Employee was in the process of lifting wire mesh panels into a pickup bed for transport the wire mesh panels are 4ft by 8ft and weigh approx. 10 lbs each.

Drilling using a jackleg when the steel broke. The machine shot forward causing his hand to strike the broken steel resulting in a fracture to the tip of the left little finger and a laceration needing nine sutures to the left little finger.

EE was using a pressure washer when a surge of pressure caused a loss of control of the wand resulting in the tip going over and cutting through his boot.

EE was operating a 740 Cat Haul Truck on frozen surface of tailings disposal site. He hit a bump and his seat bottomed out jaring his spine. He felt pain in his back and was taked to the Dr in Fairbanks. The Dr feels that this is a strain of the back associated with possible bulged disk. The Dr has prescribed two weeks of Physical Therapy.

About 600 tons of material fell from the back of an inactive mining area. Initial observations indicate that ground support bolts installed were not long enough for the actual ground conditions. It is estimated that an area about 20'X50'X6' fell. There were no injuries or equipment damage.

EE was installing ground support with jackleg drill, while drilling a hole, a baseball sized rock fell through the existing wire striking EE in face near corner of right eye resulting in a laceration that needed 5 sutures.

Employee placed hand into food processor vs using supplied pusher tool. Laceration to finger - full return to work - Medical reportable due to prescription medication given for pain and stitches. Employee was wearing cut resistant glove.

Employee tripped over door threshold - not carrying anything - slip resistant footwear was being worn - acute knee sprain to right knee. EE given 7 days off by MD and vicodin. Rotational scheduled return date 12/28/06.

An Excavator being operated by a contractor contacted a buried 13.8kv power cable. The electric breakers did not operate properly causing a fire in the substation and small fires in the cable trays in the mill. Some electrical switch gear was destroyed and power to the mine site was curtailed.
