Showing all 4Employee was in truck when the starter got hung up. Employee left the truck running with it in gear when he got out to fix problem. He was underneath the truck with his left foot under the tire when the starter started to work causing the truck to run over his left foot.
CRUSHER OPERATOR WAS STANDING BETWEEN THE LOADER AND THE CONTROL VAN. THE LOADER ROLLED OUT SLIGHTLY AND EE WAS PINCHED IN BETWEEN THE LOADER AND THE VAN. THE LOADER OPERATOR IMMEDIATELY BACKED THE LOADER UP, THERE WAS NO VISIBLE INJURY. WE SENT EE TO THE DOCTOR TO BE CHECKED OUT AND EVERYTHING CHECKED OUT FINE.
EE WAS REMOVING A WELDING HITCH, THE HITCH SLIPPED AND SMASHED HIS LEFT PINKY FINGER CAUSING IT TO BREAK.
