Employee was changing blow bars in crusher. After changing one bar they manually rotated the rotor. During the manual rotation a foot slipped into contact with the rotor. The weight of the new blow bar bent foot backwards causing bruising and some swelling. Dr. recommended to keep weight off of the foot for one day then return to work.

Showing all 8Subject was not feeling well due to a accident at home that happened several weeks ago. Had a nail puncture right temple area.(reason behind calling rescue squad). Subject was sitting in break room; tingling in limbs, dizzy & light headed, tunnel vision, rapid pulse rate, heavy breathing, stumbling around. Subject said he felt like he was going to pass out.
Person was attempting to threw block of wood onto conveyor, block was wet causing block to slip back and injure finger.
Employee was removing hydraulic line on Volvo loader. He had raised the bucket, remove front cover, taken 3 bolts out of line. The line then blew off then the main boom cylinder came down on top of him pinning him between differential and boom causing accident.
The crusher was out of adjustment. EE went to put in return rotor and plate flew back and hit his thumb.
EMPLOYEE WAS CUTTING BAND OFF OF ROLLED UP SCREEN. SCREEN SPRUNG FARTHER THAN EXPECTED & RIPPED A GASH IN HIS LEFT KNEE, LEFT SIDE OF KNEE CAP.
BLOCKAGE OF STEPS, BROKEN STEPS AND NO GRAB RAIL.
