While moving a lift chain, the pip it was wrapped around rolled pulling the hook through the left thumb

Wet Plant was shut down in order to remove some jammed rocks from tail pulley on cone cross over belt on wash/plant, plant was unexpected turned on and hand got caught in the belt, because of a language barrier the co-worker thought they wanted the bump over turned on to move the rock. This caused EE's hand to get caught up in screen

Operating the haul Truck on the haul road near the new hopper and conveyer, there was water and mud down near the bottom of the ramp. While driving empty the employee hit a hole and this caused the seat in the truck to suddenly slam down and caused employee's back to compress, this caused pain in back.

Was using a razor knife to cut conveyor belt and cut left thumb on top.

Driving down a road that the Haul Trucks drive, hit a patch of ice and slid into a berm turning the truck on side.

Employee was on top of water truck turned to go down the ladder, hooked my right boot under lip of tank on top of step and lost balance. Fell down backwards hitting the ladder with back on the way down and landed on right hand and left side of face resulting in a broken right wrist and cut above my left temple.

Injured person is a non-Varra employee. He works for Devoe Contracting. He was waiting to be scaled out and had a heart attack. 911 emergency was called and the flight for life was called by EMT and so was the fire department. Then he was taken to Greeley medical center for further treatment. NOT A VARRA EMPLOYEE

Had some packed snow on the ground. EE did not notified anyone that he had slipped. He continued to work until 2/5/2014 when I received a call from the hsp that he had been admitted because he slipped and hit his head. I then filed a claim with our Workers Comp Carrier. We are still not sure if he fell at work, that is being investigated by the Workers comp Carrier

Miner left vehicle running and in drive. Miner exited the vehicle. Vehicle started rolling. Miner opened door to stop vehicle. Vehicle hit concrete block. Steering wheel spun, miner's right forearm/wrist got entangled in the steering wheel. Sprained right wrist.

Lifting screens from squeeze sreen, he strained his lower back. (x-ray only, restrictions, anti-inflammatory muscle relaxant)

During regular work-wrist started hurting-tried to lift large dump of mud.

EXCAVATOR BUCKET DROPPED INTO HAUL TRUCK BED, SOIL UNDER TRACKS GAVE WAY ALLOWING BUCKET TO DROP INTO TRUCK

Helping unload equipment from truck hurt right shoulder when climbing on to truck.

Foot slipped on step when exiting haul truck hurt right foot instep walking boot.

Replacing v-belt on pulley-belt caught left pinky finger. cracked small finger-left hand finger x-rayed-a tape splint was applied.

Repairing hydraulic pump on 980 Cat loader. Foot slipped, lower left leg sustained contusion. Diagnostic x-ray, Celebrex for pain.

HE STRAINED LOWER BACK MUSCLE OPERATING SCRAPER. PRESCRIPTION DRUGS ORDERED ON FIRST VISIT.

OPERATING DOZER, ROLLED UP ON ROCK AS HE WAS BACKING, BRUISED SIDE ON SEAT.

STEPPED FROM HAUL TRUCK, TURNED TO LEAVE AND TWISTED RT HIP, STRAINED RT HIP MUSCLES. DR PRESCRIBED MUSCLE RELAXANT.

STEPPED OUT OF CAR, SLIPPED ON EARLY MORNING ICE, FELL ON HIS POSTERIOR CAUSING HARM.

EAR INFECTION, AN ALERGIC REACTION TO EAR PLUGS.DR. PRESCRIBED EAR DROPS FOR 2 WEEKS.

USING 3# HAMMER ON 10" SNAP ON CHIESEL-SMALL PIECE OF CHIESEL BROKE OFF, PUNCTURED, LEFT INDEX FINGER. LEAVING PIECE OF METAL EMBEDDED.

CHANGING OIL IN CAT D300E HAUL TRUCK BUMPED OIL PAN WITH WRENCH. FOREIGN MATERIAL FELL IN EYE. (TREATMENT ANTIBIOTIC DROPS.)

REMOVING HYDRAULIC LINE FROM CYLINDER- PRESSURE WITHIN BLEW OIL OUT OF LINE- STARTLING EMPLOYEE. HE JUMPED BACK HITTING HAND ON EQUIPMENT.

MUD PICKER STATION- A HOLE ABOUT 10" LONG AND 2 1/2" WIDE ON GUARD. EMPLOYEE STEPPED FORWARD TO STEADY HIMSELF AND HIS HAND WENT THRU THE HOLE AND FINGER WAS CAUGHT BETWEEN ROLLER AND BELT.

GREASING LOADER- WENT UP TO CHECK ON ZERK- HAD GREASE ON SHOE- SLIPPED AND FELL FROM MACHINE (ABOUT 5 1/2-6FT.).
