EE backed up to berm to dump fill over embankment when haul truck went over the berm and down the embankment. EE had several bruises and broken ribs.

EE was using chain hoist to lower cylinder from excavator, when the cylinder moved quicker than anticipated, pinning their right hand between the cylinder and the chain. Their fingers were cut when they pulled their hand away.
Rock fell from chute onto employee's right hand causing a laceration to index finger.
Employee stepped up on a rock when their foot slipped cutting their right shin on the rock. Employee's leg was cut and they received stitches.
EE hurt right shoulder when lifting steel ladder over head to place into truck. Lost time not incurred until 2/22/2023.
EE was in the hopper working on the warrior screen. EE stepped up and out of the hopper, onto what EE thought was steel, but turned out to be rubber. EE fell approximately 6 feet onto a pile of rocks, similar to light stone fill. EE bruised EE's right leg above the knee/thigh area and scraped the back of EE's right arm.
Employee was repairing a screen in a tight space. As a result of the tight space, employee's hard hat was knocked off and EE hit EE's head while exiting the screen. Employee's head was lacerated. EE received staples at WellNow Urgent Care.
Employee was climbing in and out of a screen when employee twisted left hip and knee, resulting in pain. Employee did not lose time until 12/12/2018.
EE is experiencing sciatica pain down EE's leg and believes it is due to EE's regular work duties, such as driving the Euclid Haul Truck and/or performing shoveling/cleanup around the conveyor. EE's first day of lost time was 6/28/18; however, it wasn't until 7/6/18 that EE reported that EE felt the injury was work related.
The employee slipped on snow/ice. Employee grabbed a piece of equipment to prevent falling and injured the right shoulder in the process. The employee did not receive "medical treatment" until 5/9/18.
The employee was welding in a chute on approximately 1/3/18. The snow and ice that melted as a result of the welding refroze on the ground. The employee slipped and fell on right elbow, injuring right elbow and right shoulder. The employee's first day of lost time did not occur until 6/14/18.
Employee was standing on frame of Trommell Power Screen prying on a belt scraper to adjust it. The bar slipped and hit the employee in the face causing them to fall to the ground, where they sustained injuries.
The employee was opening the tailgate to EE's truck, when EE's feet slipped, causing EE to pinch EE's fingers in the tailgate. This resulted in lacerations to EE's right index finger and right middle finger and a bruise to EE's right ring finger.
The employee was loosening a frozen/stuck bolt on a crusher with a wrench when EE strained/injured left shoulder. The employee did not receive "medical treatment" until 6/7/16.

Customer was driving loaded dump truck down a grade when the truck went out of control. He jumped from the cab and was run over by the truck. Truck involved in accident was owned by a customer, not a contractor or mine operator.

Employee was placing cinder blocks under job trailer (office) and used right shoulder on underside of trailer to lift trailer slightly to fit more blocks underneath for trailer's support.
Non employee and another non employee were tresspassing on quarry property. After repeated failed attempts, their vehicle mounted the berm, descended the interior, and fell to quarry floor. Driver died at the scene, passenger died after transport to local hospital.

EE WAS DESCENDING FROM EUCLID TRUCK VIA LADDER TO THE GROUND. WHEN THE EE REACHED THE GROUND, SHE LOST HER FOOTING, AND SPRAINED HER LEFT KNEE.
EE WAS USING GRINDER TO SMOOTH PIECE OF METAL. METAL SHAVING ENTERED LEFT EYE.
EE WAS WORKING WITH RIGHT ARM IN 10" PIPE TO RETRIEVE FALLEN TOOL. FAR END OF PIPE FELL, CAUSINGNEAR END WITH ARM INSIDE TO RISE AND PINCH EE ARM BETWEEN RIM OF PIPE AGAINST PIECE OF STEEL RESULTING IN BRUISE TO ELBOW AREA OF RIGHT ARM.
STONE DUST GOT INTO BOTHE EYES OF EE WHILE DRIVING EUCLID TRUCK ON HAUL ROAD, IMPAIRING VISION.
