Showing all 5Employee operates a wheel loader - The seat in the loader has air ride - the seat was starting to not hold air - when it would lose air it would take about 10 min to air back up - so while mining the person jarred their back and had to leave work to see a doctor. Other people had ran this loader the week before with no complaints about the seat not airing up.
Employee's left arm was trapped or pinned & shoulder was dislocated when cutting a bracket on the grizzly feeder. The feeder came down and entrapped his arm between the feeder and the hopper.
Employee was pressing the start button for the slurry pump when the electrical panel exploded. He received burns on the neck and throat and a burn on right arm. Investigation found no determinable cause as panel was destroyed. Possibly a short or an arc thru the air. All electrical panels were inspected by hipsage electric.
Employee states extreme discomfort with his right knee. He had a hard time with a specific injury date. Employee is a material handler with a loader blender. He remembers hurting his knee when getting down from the loader.
