Employee was spraying off concrete pad under Jaw Crusher. He said he slipped and sprained his ankle. He said he didn't need to go to the doctor, he just went on home. On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.

Showing all 21EE was using a water hose to spray off a concrete pad. He bent over to place the water hose on the ground and as he stood up he felt a pain in his lower back.
Employee was using a water hose to wash a concrete pad. He stepped off the concrete pad (3" elevation change) and lost his balance. He fell to the ground and struck his hand on a sharp rock causing a small laceration which required stitches.
Employee was attempting to adjust a dozer's track roller by striking it with a sledge hammer when a piece of metal from the roller struck him in the lower torso. The injury required 3 stitches.
Employee complained of back pain after attempting to move wooden blocks.
EE told a supervisor he felt a pull in groin. The details didn't appear to be something that could be work related so the supervisor told him he could leave. The ee told the supervisor later that he had pulled something in his groin while moving a screen. Information obtained from the doctor indicates this could be a recordable inj due to conflicting MD information.
Miner was climbing the ladder to his truck and turned to look at another miner. As he turned, he felt a "pinch" in his back. The injury was first reported by the miner as a NON-WORK RELATED INJURY. On 10/12/10, the miner claimed the injury as work related to the Safety Director.
Employee was cutting a zip-tie from a hydraulic hose and lacerated his thumb. The laceration required 5 stitches.
A maintenance employee was working with two pieces of steel when his finger became wedged. The pressure caused his finger to fracture.
Employee was walking beside a handrail and felt a stinging sensation on his finger. He looked at the finger and noticed blood dripping from a finger. He had apparently hit a metal weld burr and cut his finger.
Inspecting a screen he twisted his knee.
An employee was using a box cutter knife to cut a piece of belt and the knife cut his arm.
The employee was attempting to turn a valve and felt a "pop" in his right knee. The injury was diagnosed as a sprain.
EE COMPLAINED OF LOWER BACK PAIN & WAS SENT TO MEDICAL CENTER WHERE HE WAS DIAGNOSED WITH A LOWER LUMBAR STRAIN. THE SEAT OF THE LOADER HAD BEEN ADJUSTED TO A POINT WHERE THE SPRING NO LONGER PROVIDED SUPPORT.
EMPLOYEE BEGAN TO FEEL BACK PAIN ABOUT SIX HOURS AFTER THE WORK SHIFT BEGAN. HE ADVISED THE SUPERVISOR OF THE BACK PAIN AND WAS INSTRUCTED TO SEE A PHYSICIAN IF NECESSARY. HE WAS DIAGNOSED WIT H LOWER LUMBAR STRAIN.
EE'S LEG CONTACTED CONCRETE. AN IRRITATION FOLLOWED. THE IRRITATION BECAME WORSE AND BLISTERS FORMED AT CONTACT AREA. 2ND DEGREE BURNS. EE REPORTED INJURY ON 9-25-02.
EMPLOYEE STEPPED ON A PIECE OF STEEL AND TWISTED HIS ANKLE. HE FELT A PAIN IN HIS BACK.
EE WAS CHANGING CRUSHERS BARS, EE STRUCK BAR HIT HAMMER AND A PIECE OF METAL BLEW OFF THE BAR AND STRUCK THE INJURED EE S HAND. EE RECEIVED TWO STITCHES.
A PIECE OF STEEL ROLLED OFF THE FORKLIFT FORKS AND STRUCK THE EE'S FOOT. THE EE WAS WEARING STEEL TOE BOOTS BUT THE WEIGHT OF THE STEEL PRESSED THE BOOT STEEL AGAINST HIS TOE.
EE WERE REMOVING ROCK FROM UNDERNEATH THE CRUSHER. ANOTHER EE DROPPED A ROCK ON THE INJURED EE'S HAND.
EMPLOYEE SLIPPED ON A PATCH OF ICE. HE GRABBED BUG SHIELD TO AVOID FALLING AND STRAINED HIS SHOULDER, EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 1-27-2000.
