While descending the ladder on the screen plant, foot slipped off lower rung. Caught self with left hand causing shoulder strain and sprained EE's left foot/ankle when it hit the ground.

Employee was standing on the ground working on a part of a VSI that was suspended approximately 6" in the air. The part fell on employee's left foot breaking a small bone on the top of EE's foot.

Laborer was passenger in other laborers personal truck. They were driving on an approximately 1 1/2 mile hilly dirt road. The driver met a drilling machine on the road, hit brakes, lost control of truck, went off the road and rolled truck. The posted speed limit was 15 mph. The driver said EE was driving 35-40 mph.

Laborer was driving personal truck with another laborer to go and pick up the water truck. It was a dirt road approximately 1 1/2 miles long. The driver met a drilling machine on the road, hit the brakes, lost control and went off the road and rolled the truck. Speed limit was 15 mph. Driver said they were driving 35-40 mph.

The miner was trying to locate a leak in the oil cooling system and removed the guards for better access. The diesel engine was started to initiate the leak and the miner made contact with the moving fan blades with a flashlight they were holding. The miner's hand was struck by the moving fan blades causing serious injury.

Employee was shoveling built up materials on the catwalk around cone crusher. A baseball size rock that was on the lip of the basket surrounding the cone feed, vibrated off the lip and fell onto the employee. The rock struck employee right in the middle of his back while he was bent over, shoveling.

Employee was walking while looking at a number on a fuel filter. He was not paying attention to where he was walking, tripped over an exposed piece of rebar, and landed on another piece of rebar. Injury sustained was a small puncture wound to his right forearm.

Employee was raking up rocks around a beltline as the plant was preparing to shut down for the end of the shift. Employee went to pick up a rock on the ground and another rock bounced off the conveyor belt and hit the top of his index finger on his left hand which caused the tip of the finger to split open which required stitches to close.

Employee was helping to install a chute on a 6x20 screening plant. He was using a prybar to help align the chute when the chute shifted and his right ring finger got caught between the chute and the screening plant. His finger was fractured. He saw a doctor on 5/23/12 and was released to full duty. Following his doctor's appointment he went on his scheduled vacation.

Employee was walking from his car when he twisted his left ankle on a rock that was approxamatly 1.5 inches tall by 3 inches wide.

Employee was cleaning the head pulley shaft with a pneumatic sander when disc flew off and struck employee both the left and right forearm. The employee returned to work the same day with no restrictions determined by Foothills Walk-In Medical Care. No loss of time from work.

Hitting a loader tooth with a hammer (in an attempt to loosen and remove the pin), piece of steel broke off of tooth and punctured right thigh. The fragment was 5mm x 11mm. Worker received two stiches.

While cutting a conveyor belt with a razor knife, employee cut his upper right leg with knife. The wound was approx. 1/2" and required two stitches.

While working on the cone crusher top assembly, employee A was holding bolt with wrench. Employee B was loosening nut with an impact wrench; when torque was applied to the nut employee A lost grip on wrench causing wrench to make contact with employee B's face resulting in damage to mouth requiring five external stitches and 2 internal stitches.

While working near the jaw crusher a rock fell and hit the miner a glancing blow to the left shoulder. The primary contributing factor was the miners location - too close to the jaw. After a trip to the clinic on his way home, it was determined that the only damage was bruising. The miner returned to work the next day to regular duty. There was no time loss, but he saw a doctor.

While setting kicker in the correct position, the legs collasped quicker than anticipated striking the employee's right arm and knocking him to the ground. Employee suffered a bruised right arm.
