Employee slipped and fell while walking on the ground. The ground was covered with ice and snow. The area where slip and fall occurred was snow covered and the surface of the ice was not exposed at the time of the accident. The employee broke his leg during the slip and fall accident.

Showing all 16Employee was guiding and directing loader operator to move 150' Stacker, was watching loader and stepped on tin cover for conveyor that was on the ground and cover flipped up and struck his face.
Steel hatch door was opened to access hydraulic equipment on classifying tank station. Door was propped up against rail in verical position but was not secured. Wind blew door closed on EE's lt hand. No equipment was damaged. Injury to hand was swelling & bruising. He was taken to hospital to have injury inspected. Xrays were neg. Splint was applied to wrist & arm. No impairment.
Contractor was on site to work on electrical system at new office/scale house. The victim reportedly passed out while working. 911 was called & First Aid/CPR was administered. Victim was taken to hospital by local EMS @ approx. 2:30pm. Victim was not employee of SMC. Mine was not in operation at the time & power was not hooked up. Victim died of Heart Attack.
Employee was trying to tighten hose fittings on backhoe tractor with crescent wrench. Wrench slipped off and struck him in face. Cut cheek.
EMPLOYEE HAD HIS THUMB HIT BY A CHAIN WE WERE USING TO ADJUST LIGHT TOWER AT 125 OF PRIMARY. THIS TOOK OFF HIS NAIL & CUT THE THUMB SO HE WAS GIVEN 9 SUTURES.
EMPLOYEE CUTTING OLD SPLICE OUT OF BELT AND SLIPPED WITH RAZOR KNIFE CUTTING HIMSELF ON LEFT HAND.
Truck was travelling on downgrade travelway into mine.Operator lost control of speed and had to steer truck into safety berm to stop.Resulted in minor injury and damage to truck.Setbelts were worn.Contributing factors may have included;truck travelling too fast at approach to downgrade slope,proper gear not used,truck fully loaded,excessive use of air brakes.
EE WAS HIT BY FALLING ROCK AT SECONDARY PLANT WHILE CLEANING UNDER SCREEN UNIT. ROCK HIT SIDE OF SAFETY GLASSES AND CUT SIDE OF HEAD. EE WIPED EYE WITH HAND AND GOT AGG. DUST IN EYE, SCRATCHING EYE.
CHANGING OUT TROUGH ROLLER, EE REACHED IN TO MOVE ROLLER AT THE SAME TIME EE WAS MAKING ADJUSTMENT WITH SLEDGE HAMMER.
TRUCK DRIVER WAS UNLOADING MATERIAL TO ADJUST HAUL WEIGHT AFTER DUMPING SOME MATERIAL OFF. DRIVER FOUND A ROCK STUCK BETWEEN THE TAILGATE & BED, SHE REACHED IN & PULLED ROCK OUT CAUSING THE TAILGATE TO CLOSE ONTO HER RIGHT HAND.
CRUSHER OPERATOR CLEANING UP SPILLAGE MATERIAL. HE TWISTED TO SIDE AND THREW BACK OUT OF PLACE.
EE 2ND SHIFT FOREMAN-GRINDING ON SHAFT OF SNUBERROLLER, GRINDER CAUGHT AND SPUN AROUND AND CUT RIB ON RIGHT FOREARM. SENT TO HOSPITAL AND RECIEVED 4 STITCHES.
REMOVING HYDRAULIC HOIST CYLINDER OFF R-50 EUCLID TRUCK. PRESSING TOP HOIST PIN OUT TO REMOVE CYLINDER, PIN POPPED OUT & CYLINDER TILTED BACKWARDS PINNING LEFT FOREARM AGAINST REAR DIFFERENTIA L.
DRIVER SUPERVIOSR CONTRACTOR/TRUCK DRIVER FOR SYLVESTER MATERIAL CO, WAS OUT OF TRUCK AFTER BEING WEIGHED. HE LEANED ON GUARD RAILS AT SCALE AND RAILING BROKE, THUS FALLING BACKWARDS, STRIKING HIS BACK. THE RAISED PLATFORM AT SCALE/OFFICE IS 18" HIGH.
EMPLOYEES WERE REMOVING THE BACK PLATE OF THE NORDBERG SCREEN PLANT TO INSTALL NEW SCREENS. THEYUNBOLTED THE PLATE AND STARTED TO REMOVE THE PL ATE WHEN IT SLIPPED OUT OF THEIR HANDS RESULTING IN A BLUNT FORCE INJURY TO EEMPLOYEE'S FOREARM WHEN IT FELL.
