Employee was working with a co-worker to take a wear plate off the screen at the crusher plant. EE placed EE's thumb on top of the carriage bolt as the co-worker used the air gun to loosen the nut on the other end of the bolt. The bolt popped up out of its groove and sliced EE's right thumb.

Showing all 4EE was dismantling the crusher and releasing a binder on a chain that was holding a conveyor stable. He put a lever on the chain binder to release it and was repostioning himself when it popped free and threw the lever which struck him on the cheek.
