Driver came up to stock pile. He opened gates on trailer. Wheels of trailer caught and truck and trailer turned on right side.

Operator was driving down the road. He went to pass oncoming vehicle on the right and hit a hole and bounced out of his seat. He landed half way back in his seat.

DUE TO RECENT RAINS AND THE EQUIPMENT BEING USED, GROUND WAS WET AND MUDDY. DAMAGE WAS DONE TO RIGHT FOOT. THE PRESSURE FROM THE HOSE CUT HIS BOOT AND THE TEMP FROM THE HOSE CAUSED 3RD DEGREE BURNS TOT HE TOP OF THE FOOT.
