Employee was using a Case SR270 Skid Steer to remove material spillage from under conveyor. Employee failed to ensure beaver tail attachment was secure on front of skid steer and when they attempted to load with materials, it slipped off of the front plate and catapult towards the cab cutting hand.

Employee was sitting in haul truck waiting for Trac hoe to load with boulders, when a boulder was dropped on tail end of truck causing the bed to raise and then slam down rocking the cab hurting employee back
Employee was tightening a bolt on Cat D-9 Dozer track return idler that EE noticed had fallen out. While pulling on a wrench, felt a sharp pain in right shoulder. EE continued to tighten the bolt and then went for some oil to put in dozer and when EE picked up a (5)five gallon bucket of oil that had about (4)four gallons in it EE states pain in shoulder was worst.
Employee had been tightening Jaw bolts on a Telsmith 30x50 crushing jaw. After completing task he was leaving area were working when he went to place his foot on an I-Beam to assist his dismount, he misjudged where to step and fell striking his head on the brake cam rod causing a laceration to the head. He was wearing his PPE (Hard Hat) which fell off when he bent over.
Employee was assisting co-worker putting a guard back on a tail pulley. Co-worker had to use a sledge hammer to seat guard and when he struck guard hammer head came off striking employee in left arm.
Employee was cutting a 7'long x 12" wide piece of 3/8" AR Steel(approx 150lbs). When he was turning it over to begin grinding the edges, He lost his grip with right hand and tried to prevent Steel from falling, causing his fingers to be crushed between the steel plate and the welding trailer fender.
