Miner was unloading screens - glove on right hand snagged and caused sharp screen end to lacerate right ring finger.

MINER WAS USING A HAMMER TO REMOVE A RIPPER POINT. MINER STRUCK THE TOP OF THE RIPPER POINT CAUSING A PIECE OF THE EDGE TO SHEAR OFF AND BECOME A FLYING OBJECT. FOREIGN BODY ENTERED THE MINER'S RIGHT KNEE, LODGING JUST BEHIND THE KNEE REQUIRING SURGERY AND OVERNIGHT HOSPITALIZATION.

MINER WAS PRYING A VIBRATORY HOPPER WITH A BAR TO REMOVE ROCKS. MINER WAS STANDING ON A LOOSE MOUND OF DIRT WHEN THE BAR SLIPPED CAUSING MINER TO LOSE FOOTING. MINER WAS WORKING INSIDE THE CRUSHER HUT STRUCTURE AND TRIED TO STEP THROUGH FRAME ONTO LOOSE MATERIAL AND FELL. MINER STRUCK CHEEK AND CHIN AGAINST THE STRUCTURAL STEEL.

Employee was checking around the crushing plant, when he got some dust and dirt in his right eye. The next morning it was swollen shut and extremely painful. He went to work and his supervisor told him to rinse with saline solution and to stay inside. His supervisor also put a patch over it.

Employee working on truck with hood tilted forward, wind caught hood and closed on victim's head cutting head and compressing neck; victim had hard hat blown off just before hood came down. 1"-2" cut on top of head and complaint of stiff neck.

Operator said that the truck stalled as he was leaving loading area and he was unable to maintain control of vehicle. Vehicle overturned, operator put hand outside of window and arm was caught under mirror and mirror bracket that were crushed during rollover causing hand to be de-gloved and to break arm in couple of locations.

Worker was using a 1" air powered impact gun while changing cutting edges on a CAT 966 loader bucket. Worker placed his left palm on bolt head and engaged the impact gun, the bolt spun and cut palm of left hand.

Worker was using hand grinder to shape piece of metal. After 15 or 20 minutes of grinding a small piece entered worker's right eye causing irritation. The piece was secured to work bench. The worker is experienced at using such equipment and was wearing proper eye protection.

Worker was servicing a wheel loader. He had his left foot on a step and shifted his weight to the right foot on the rear fender. The fender was covered in dirt and he slipped off the equipment to the shop floor. He injured his left knee upon hitting the floor.

Employee was using a 1-1/16" combination wrench on the same size nuts to tighten the clamp rails on a screen shaker. While applying downward pressure to the wrench, employee said he felt something give way in his left shoulder. Since then, employee has had difficulty using his left arm and shoulder.

Employee claims he re-injured his back from another injury at another one of our locations in the past. This injury was never reported at the time. We can find no evidence to support employee's claim that he sustained a work-related injury. We have statements from other employees and supervisors stating that at no time did they see or hear employee say he strained his back.
