EE WAS TAKING A LEAF SPRING OFF OF 22 DIESEL. HE WAS LOOSENING U BOLTS ON REAR END. WHEN HE TOOK THE LAST NUT OFF THE SPRING DROPPED AND THE REAR END FELL ON HIS LEFT HAND, CAUSING A CUT BETWE EN THE INDEX AND MIDDLE FINGER.
EE STATED THAT HE AND THE OTHER OPERATOR AT #3 MINER HAD JUST MOVED UP AND GOT MINER READY TO CUT. EE WENT BACK TO HIS CHUSHMAN VEHICLE AND SAT DOWN AND FELT SHARP PAIN IN HIS LOWER BACK.
EMP WAS DRILLING A HOLE IN A 12V. (SMALL) LIGHT ASSEMBLY. SAID HE FELT DRILL CATCH A LITTLE SO HE LOOSENED HIS GRIP SOME. WHEN HE DID THE LIGHT ASSEMBLY SPUN AROUND WITH THE DRILL CUTTING HIS LEFT THUMB. (LIGHT ASSEMBLY IS WHAT CUT HIS THUMB). THUMB REQUIRED THREE SUTURES.
EMP BUMPED HIS HEAD ON A 2 X 4 THAT WAS NAILED TO THE BACK, JAMMING HIS NECK. (THE BACK WAS 56"HIGH). AFTERWARDS HE GOT ON #3 KUBOTA TO DRIVE TO THE FEEDER WHERE HE RAN OVER A HIGH CUT AND JAM MED HIS NECK INTO THE BACK AGAIN. EMP FIRST DAY OF RESTRICTED DUTY 4-26-96.
EMP WAS MOVING BOLTER CABLE BY X-FEED JACK ON A PCA MINER AND STEPPED ON A ROCK AND TWISTED LEFTKNEE AND FELL AND HIT LEFT KNEE ON X-FEED JACK. RESTRICTED DUTY STARTED 5-22-96.
TOTAL POWER FAILURE AT OPERATION, CAUSED BY FAILURE SYSTEM WIDE AT S.P.S. TIME OFF WAS 9:30 AND BACK ON AT 3:15 PM 4-16-96.
EE WAS BACKING DOWN 6 1/2 BELT ENTRY. HE WAS TURNED AROUND LOOKING OVER HIS RT SHOULDER WHEN HE GOT TO CLOSE TO THE RIB ON THE LEFT SIDE. HIS FOOT WAS HANGING ABOUT 1/2 WAY OUT AND GOT CAUGHT BETWEEN THE RIB AND RIG HD WAS DRIVING. RESTRICTED DUTY 3-12-96
EMPLOYEE WAS BETWEEN 2 & 3 GRANULAR ROUGHER CELLS, WASHING AREA DOWN WITH WATER HOSE. HE SLIPPED ON A JET OR SIMILAR OBJECT. HE DID NOT FALL DOWN, BUT FELL BACKWARD AND FELT A PULL IN HIS LEFT SHOULDER AND PAIN IN HIS AND NUMBNESS IN HIS FINGERS.
EMPLOYEE WAS SHOVELING A SPILL AT A TAIL SECTION. WHEN PULLING BACK ON SHOVEL THE HANDLE HIT HIM IN THE MOUTH. (BROKE PART OF ONE TOOTH OFF)
EMPLOYEE WAS WORKING ON BUGGY CABLE REEL. HE HAD THE COVER UP TRYING TO GET CABLE LOOSE FROM REEL. THE COVER FELL DOWN AND HIT HIS RIGHT HAND. FIRST DAY OF RESTRICTED DUTY 12-24-95.
UEE WAS SCALING RIB, WHILE SCALING HE BROUGHT DOWN A PIECE OF TOP AND A OID POWER CABLE ABOUT 1" IN DIAMETER. THE CABLE HIT HIM ON THE BACK OF THE NECK.
LOADING ROLLS OF RUBBER ON FLATCAR WITH FORKLIFT. TILTED FORKS TO UNLOAD RUBBER, THE ROLL OF RUBBER CAME BACK TOWARDS HIM, RAN TO TRY AND GET OUT OF THE WAY. THE ROLL OF RUBBER BOUNCED TOWARDS HIM AND PINNED HIM AGAINST THE RIB.
EE WAS PULLING PANEL BETL TAILS WITH #2U2D. HE DIDN'T SEE THE WINCH LINE THAT WAS PULLED TIGHT AND TRIPPED. HE FELL DOWN AND SAID THAT HIS RT SHOULDER AND LEFT HAND WERE SORE.
EMPLOYEE WAS TRAMMING ROOF BOLTER AND TRAMMED ROOF BOLTER ON HIS LEFT FOOT.
SHUT OFF UPPER REDDLER WITH PUSH BUTTON PENDENT. REPAIRE DREDDLER FLIGHT JOGGED UPPER REDDLER WITH PENDENT OT NEXT BENT FLIGHT. WHILE WORKING ON SECOND BENT FLIGHT UPPER REDDLER STARTED.
SHE WAS USING FRESH WATER TO WASH OUT DISCHARGE LIME SHE OPENED INTAKE TO PUMP AND WASHOUT HOSE BLEW OFF SPRAYING HER WITH HOT BRINE.
OLD INJURY - REOCCURRANCE - 1ST RESTRICTED DUTY 5/31/95
ON 9-6-92, EE SUFFERED AN ON THE JOB INJURY. HE THEN BECAME LOST TIME 4-4-93. RETURNED 6-23-93. AS OF 2-2-95, EE IS ON RESTRICTED DUTY. THERE WAS NO NEW INJURY, JUST AN AGGRAVATION OF HIS PREV IOUS INJURY.
WORKING WITH COWORKER TO FREE UP BY PASS DRIVE ON THE UNDER FLOW WRENCH SLIPPED CAUSING EMPLOYEE TO SLIP ALSO 1ST DAY RESTRICTED DUTY 04-05 -95
EE HAD JUST BEAT OUT CHUTE TO CP FEED PUMP - GOING DOWN STAIRS BANGED KNEE ON STEP.
TIGHTENING BOLTS ON THE HEAD OF #19 MINER. WHEN THE WRENCH SLIPEPD HE FELL BACKWARDS.
EE HAD RIGHT LEG OUT OF CAB OF MINER, BACKING MINER UP. HE RELEASED TRAM LEVERS AND MINER KEPT COMING BACK AND PINNED FOOT UNDER MINER & BETWEEN RIB.
EMPLOYEE FELT ITCHING ON BACK - WHEN HE WASHED AREA OFF NOTICED BLISTER LIKE AREA WITH SKIN PEELING OFF. SUSPECTED BROWN RECLUSE SPIDER BITE.
EMPLOYEE WAS GOING TO MAKE SPLICE ON BELT. CO-WORKER STATED THEY WOULDN'T HAVE TIME TO FINIH BEFORE QUITTING TIME, THEY PUT CLAMPS ON BOTH ENDS OF BELT. ALL THAT WAS HOLDING BELT TOGETHER W ERE 3 CLIPS. EMPLOYEE WAS THROWING HALF HITCHES WITH A CHAIN, CLIPS GAVE WAY. R IGHT 4TH & 5TH FINGERS HUN UP BETWEEN CHAIN AND ROPE.
EMPLOYEE WAS PUSHING ON REEL OF WIRE WHEN HE FELT A PAIN IN GROIN.
EMPLOYEE HAD FEET TANGLED UP IN SME CABLE IT CAUSED HIM TO FALL AS HE WAS GETTING OUT OF HIS VEHICLE.
EMPLOYEE HAD BLASTED THE INTERIOR OF THE VESSEL AND WAS PREPARING TO PAINT WHEN AN EXPLOSION OCCURRED.
WALKING AROUND DESK WHEN SHE TURNED TO ANSWER COWORKER FELT PAIN IN LOW BACK
PRELIMINARY INDICATIONS SHOW THAT #1A BELT CAUGHT FIRE AT THE DRIVE AFTER APPARENTLY BOGGING DOWN AT THE TAIL & FAILING TO KICK OUT PROPERLY DAMAGE IS APPROX 280FT OF BURNED BELT (560FT INCLU DING TOP & BOTTOM)
MOVING UP MCKNIFE PUSHING ON STEERING ARM WHEN STEERING ARM BROKE AND HE FELL AGAINST FRAME.
EMPLOYEE WAS SHOVELING WALKWAY WHEN HE TURNED CAUSING SHARP PAIN IN LOWER BACK.
APPEARS HYD FLUID TANK LID VIBRATED OFF AND SHOT HOT FLUID AND THE FILTER OUT OF THE TANK SPLASHING HOT FLUID ON EE EE ALSO STRAINED RIGHT SHOULDER WHEN HE JUMPED OUT OF CAB.
EMPLOYEE WAS STEPPING DOWN OFF TAIL PIECE OF PANEL BELT- TWISTED RIGHT ANKLE.
RAIL LOADING 4 LOADER DOWN NUMBER ONE TRACK. EMPLOYEE SLIPPED AND RAIL CAR RAN OVER FOOT.
MOVING LONG TOM TO MAIN TRACK WHEN REAR TIRE HIT BOULDER HE TRIED TO AVOID STEERING HANDLE BY JUMPING OUT OF WAY TWISTED RIGHT KNEE.

PULLING ON WELDING LEADS LEAD GOT STUCK WHEN HE JERKED ON IT SHOULDER POPPED
MOVING MCKNIVES WHEN HE WENT TO NIP ON RUBBER ROPE HE STEPPED ON IDLER THAT WAS PARTIALLY COVERED IN MUCK THIS CAUSED HIM TO TWIST HIS ANKLE
CLIMBED OVER BELT BELT STARTED UP AND HE HIT HIS KNEE ON BELT STAND.
ATTEMPTING TO STRAIGHTEN IDLER. HE HAD ONE SIDE LOOSENED AND WHEN HE LOOSENED THE OTHER SIDE THE IDLER JERKED ITSELF STRAIGHT AND HIT HIM.
DRIVING CUSHMAN LOADED WITH T-STULLS- DROVE CUSHMAN AROUND SHUTTLE CAR AND HIT SIDE OF SHUTTLE CAR WITH T-STULLS. T-STULL SWUNG AROUND AND HIT HIM.

EE AND CO WORKER WERE WORKING ON CRUSHER EE WENT TO STATION LEVEL FOR SOME RAGS AND MORE TOOLS ON 2ND LONG FIGHT OF STAIRS HE MISSED A STEP FALLING TO NEXT LEVEL.
CHANGING OUT BITS ON DRUM SET PEEN HAMMER AGAINST BIT AND HIT IT WITH 4LB HAMMER. PIECE OF METALENTERED LEFT THIGH.
LOADING BARREL OF OIL INTO 105C BARREL SLIPPED &CAUGHT HIS FINGER BETWEEN FRAME & BARREL.
JUMPED ON TO DOUBLE TOM--SELF RESCUER JABBED HISSIDE. FIRST DAY RESTRICTED DUTY--4-20-94.
