EE was injured while preparing S31 for moving from bottom of the Hematite Pit using the motivator. A cable crew was pulling slack and loops out of trailing cable. The trailing cable became taut contacting the ee causing a fall that resulted in injuries. The ee was transported to a hospital for evaluation and treatment of injuries, and later hospitalized resulting in Lost Time.

Employee was inside LN12 primary mill performing lining. After moving a rock out of the way, employee stood back up and when doing so contacted a small shard of steel on the duckbill of the liner handler with forearm resulting in a laceration. Employee was seen by nurse and later received sutures at local hospital. Employee returned to work following treatment.

Employee was grinding on steel and felt something was in employee's right eye. There was debris falling from above. Employee did not report this until the next day.

Warehouse ee was cutting cardboard, at end of shift, with a box cutter so cardboard would fit into compactor when blade of box cutter contacted employees hand resulting in a cut. Employee was taken to UP Health System Bell for medical care. Employee received seven stitches. Employee was returned to work with no restrictions. Gloves were not worn, at time of incident.

While an employee was loosening house bolt on S33 with an impact, some debris got into eye. EE tried to flush debris from eye on site. EE was brought to the hospital where a foreign body was remove from eye and employee returned to work. Safety glasses were being worn at the time of the incident. Prescription eye medication ordered.

IP was in process of completing a delivery to customer site. Foot became tangled in rope between wheel chocks causing IP to fall. IP place arms out to break fall and when landed caused fracture of the bone.

Mechanic sustained injury to fingers while up righting CAT #4306 electric pallet lift when hand contacted a pinch point. Employee was evaluated by site responder and nurse. Employee was brought to healthcare facility for further evaluation and treated for finger tip fracture. Safety department initiated investigation into the incident with support from the department.

Scraper Operator was operating scraper and didn't report an exhaust leak. Exhaust leak was fixed as soon as mechanic was aware of the problem and the machine was back in service before the end of the day. Employee went to the hospital at the end of shift.

Alleged: EE stated on next shift that EE got something in eye when needle scaling the previous shift. States self flushed and was wearing safety glasses and face shield at time of event. On 4-20-17, EE was evaluated in the ED and given rx medication for eye.

While conducting industrial hygiene duties at the Tilden Concentrator, employee reported stumbling and falling forward landing on right hand. A minor injury was sustained, but employee did not seek medical attention. Employee continued to have a problem with the finger and was later evaluated. On 08/24/17, employee underwent surgery and was back to work the next day.

EE was using a hand held grinder with a cut off wheel to make modifications to a conveyer belt guard when ee lost control of the grinder striking right leg just above the knee causing a laceration. Taken to local emergency room. 6 stitches required to close laceration. Returned to work with restrictions.

Employee operating #4267 forklift on Tilden 2 fan floor level with tines lowered when left tine caught the top of a floor launder channel causing the forklift to stop suddenly. Employee was seen by on-site nurse practitioner. Employee returned to work. EE developed neck pain after the incident and was eventually prescribed PT. EE complete 2nd session of PT on 05-04-2017.

EE was moving impact rollers from a pallet to a storage rack when one of the rollers moved rolling on top of left hand. EE pulled EE's hand out and felt a pop and pain in the wrist. Evaluated by physician and returned to work with restrictions.

Employee was operating a loader when operator stated the loader bucket hit something hard, jarring the operator. Employee was seen by on-site nurse and told nurse that the pain ee feels is related to older back problems from 2009 and 2011. Taken off work for 3 days then returned with restrictions.

EE reports exiting cab of cable truck when experienced right knee pain. EE claims traction soles on protective footwear may have caught on step tread causing stress to knee while turning. After conservative treatment, follow up evaluation was performed where restrictions were ordered on 2-2-17.

EE participated in several consecutive shifts of manual labor (bucket brigade) requiring repetitive motion. Employee picked up plastic pail containing scraps (approx. 10lbs). EE states felt a pop in right shoulder and experienced immediate pain down the outside of upper arm. Employee was evaluated by two EMTs, and then transported to Bell for further evaluation. Restricted duty.

Mechanic reported lifting a battery(about 30#)out of a loader and felt "pop" in right shoulder. Mechanic initially declined medical assessment, but later provided conservative care instructions. After a follow up assessment, restrictions were ordered on 1-30-17. Mechanic reports having removed numerous batteries recently. X-ray report showed multiple chronic changes.

Employee was installing sheeting and hit elbow on a stud on a scaffold head while pulling a piece of sheeting. No pain at the time of incident but three days later it became swollen and very painful.

The operator was being loaded when a chunk came of the bucket striking the fan tail. The operator allegedly was jostled in the cab. The operator reported it to dispatch and stated being fine. Operator reported soreness at end of shift and was seen by site responder. Instructed to follow up with nursing staff. Employee sought own medical care and was taken off work.

Employee felt strain in abdomen when removing a broken lifter from the #1 dryer exchange. It became known on 11-29-16 that the employee was diagnosed with an inguinal hernia. No work restrictions recommended.

Employee sustained laceration to right middle finger handling a warming oven. The employee was assisted by a partner in lowering the warming oven to the floor when the finger was pinched between the oven underside and floor. Employee was seen by site responders, and then brought to hospital where medical treatment was provided.

Amendment for narrative: Employee was moving two empty railcar's using the rail car puller when something on the rail car puller cable assembly caught on a metal plate. This caused the metal plate to dislodge from nesting place striking employee in legs.

Rolling rag and it caused a blister on toe. The blister caused toe nail to fall off and become infected. The employee is a diabetic. The accident report wasn't filed with the company until 2 weeks later due to the strange circumstances.

As employee moved a saw downward, the 2nd knuckle on the left hand index finger made contact with blade. Blade cut through leather glove contacting skin. Employee was seen by on-site nurse then taken to clinic where employee received three stitches and returned to work.

While removing a beam trolley, the employee was prying the wheel off with a Sleever bar when it slipped pinching the finger between the bar and the beam. The employee's left ring finger got a laceration and needed ten stitches.

EE was measuring to cut a piece of steel. The piece of steel was on a wood pallet. The pallet gave way, and the piece of steel fell on the tip of EE's right middle finger causing a laceration. The EE was evaluated by a physician and sutures closed the wound. EE returned to work.

Tilden Plant Repairman was torqueing bolts on #2 Dryer with a high torque hydraulic wrench. Employee placed hand on wrench and asked partner to apply hydraulic pressure. At this time his finger became caught in between the wrench and bull gear flange, resulting in 13 stitches. Employee is off work on administrative leave post drug and alcohol testing.

Two repairpersons were pulling the 3 CV head chute pan when it became bound on the bottom of the head chute & caused the lifting strap to break. The pan swung contacting the ee on left arm causing a laceration. He was evaluated by a Physician and sutures closed the wound. He returned to work.

EE was attempting to lift one end of wooden cribbing. EE felt discomfort to left arm once the cribbing was just being lifting off of the floor. The ee was evaluated by a Physician and given restrictions that were unable to be accommodated. Surgery is scheduled for 11-12-15.

EE was working on #3 Poly pump in the deslime basement on midnight shift and when going to retrieve a tool, he slipped and fell onto his back. He was evaluated by a Physician, given restrictions that the site could not accomodate. He was given a full duty release at his evaluation on 10-26-15.

An ee was standing nearby a pump transferring Sodium Hypochlorite 12.5% (bleach) from a bulk tote into boiler water profilers when discharge hose disconnected from pump, releasing chemical. EE was sprayed, resulting in chemical making contact with both eyes. Immediate flushing began and he was then evaluated by an Eye MD and given prescription eye medications.

A repairperson was operating a pneumatic impact inside a balling drum when material fell from the drum striking the ee causing a fall from the staging platform to lower level, about 2 ft below. EE sustained abrasions to back reporting pain in shoulder, back, & neck.EE was assessed by site MED PERS,& taken to HOSP for further eval.EE was removed from work, resulting in LOST TIME.
