Employee was climbing aboard pontoon boat from dredge when left knee gave out. Heard pop noise when this happened. After several days it was knee suffered ligament damage.

Showing all 13Employee was operating a small front end loader. While employee was climbing down loader steps they stepped on a rock with right foot and rolled ankle. Employee has been diagnosed with a sprained right ankle.
Employee was operating a front end loader. Loader traversed a bump jarring the employee and causing pain in lower back. Employee has been diagnosed with possible disc and/or muscle injury in lower back.
Employee's foot slipped on catwalk at entrance to surge pile tunnel. Fell on catwalk guardrail with right shoulder. Pain in right shoulder. Employee received some physical therapy treatments but missed no work days. Injury was later determined to be torn muscle. Surgery to correct performed on 08/17/15.
Employee was using a hammer and chisel to knock bearing loose of a shaft. The chisel bounced back and hit employee on right cheek, knocking off protective glasses and impacting the right eye. Damage to eye is unknown at this time.
Employee was repairing a truss conveyor that was laying on the ground. He had one foot inside the truss and one foot outside. While trying to get outside the truss his right foot got stuck under a truss cross member while his upper body was leaning to the outside. The result was "twist" to the right knee.
Employee was shoveling material from under a conveyor. Due to wet and cold ground conditions employee's feet slipped on ground and hit his shin on the truss of the conveyor. Injury resulted in laceration on the right shin that needed three stitches.
Employee was removing 20 lb metal flange from a pipe. Flange slipped and it pinched finger between flange and work bench. Resulted in fracture and cut to tip of 4th digit on left hand.
Employee was cutting conveyor belting with retractable blade knife. Conditions were damp. Knife and hand slipped and knife cut right thumb. Laceration required three internal stitches and seven external stitches.
VICTIM WAS REMOVING A SCREEN FROM TOP OF WASH SCREEN. ANOTHER EMPLOYEE WAS ASSISTING IN LIFT- ING SCREEN WHEN VICTIMS FINGER (RIGHT POINTING) WAS PINCHED BETWEEN SCREEN AND HANDRAIL.
EE WAS REMOVING CLAY LUMPS FROM CONVEYOR. WHILE REMOVING A CLAY LUMP EE FELT PAIN IN LEFT SHOULDER
EE HELPING REPAIR BEARING AT TAIL PULLEY OF CONVEYOR STACKER. STACKER WAS LOCED AND TAGGED OUT PRIOR TO REPAIR. WHILE LINING UP BEARING THE TAKE UP ASSEMBLY CAME LOOSE AND PINCHED EE FINGER AG AINST CONVEYOR FRAME.
VICTIM CONTACTED RACEWAY ENCLOSURE WITH WRIST AND RECEIVED ELECTRICAL BURNS WHERE HIS WRIST WATCH WAS. VICTIM RECEIVED MEDICAL TREATMENT WHERE 2ND AND 3RD DRGREE BURNS WERE DETERMINED. VICTIM SENT HOME FOR REMAINDER OF SHIFT. GROUND CONTINUITY CHECKS DONE 2 DAYS PRIOR. HOWEVER, LOOSE CONECTION FOUND IN RACEWAY.
