While in the shop working on a haul truck. The individual was removing the oil pan from the motor to replace the gasket. The oil pan shifted and pinched EE's finger causing a laceration to EE's right ring finger requiring stitches to close laceration.

Employee was installing a hydraulic cylinder when the cylinder rolled over & pinched employee's right hand little finger. It caused a laceration on the pad of the little finger & a skin avulsion on the top of the little finger. Employee was driven to the ER where 4 sutures where given in the pad of the little finger & steri-strips were applied to the top of the little finger

Employee was using a hammer to pound a pin for a vibrating screen when a small piece of debris flew into his eye. He claims that he was wearing safety glasses during this time.

Conditions of a high pressure oil leak with oxygen near the heat source of the engine on the truck were factors that created the fire. However the experience of the operator to not panic and exit the truck safely was a factor for the operator to NOT sustain any injury.

EE was lifting pipe: approximately 60-70 lbs, he felt severe pain in the groin area and a bulge appeared in area.

Twisted back while positioning idler on frame.

Pinched rt. hand small finger between handrail and screen cloth when moving; stitches.

EE while walking slipped on loose stone and twisted ankle (left). On August 19, EE went to have x-ray and learned that he had a left ankle fracture.

EE was attempting to pull a hose to a different position.

While climbing ladder employee felt something in his mid back pull; felt pain and discomfort in the middle of his back.

AS EE WAS LIFTING THE TRACK PADS OF EXCAVATOR (APPROX. 50 LBS. EACH) TO CHANGE BAD BOLTS, HE APPARENTLY TURNED OR STRAINED HIS BACK, CAUSING A GREAT DEAL OF PAIN & DISCOMFORT.

AS EMPLOYEE WAS DISMOUNTING THE TE-001 TO FUEL THE MACHINE (TRACK EXCAVATOR) HE FELT A PULL IN HIS LOWER BACK AND HIS BACK POPPED.

Employee stepped down from a truck and twisted right knee. Initially, he was returned to regular duty; he revisited doctor and was informed to have x-rays; 4/29/05; ee had 3 ligament tears. Followed up with doctor and he was told that he would be off work.

While using a cutting torch, employee's pants did not cover top of boot, and as a result some slag fell between the sock and inside boot. Initially the incident was a first aid case. On or about September 18, 2004 employee returned to doctor for a follow up visit and the doctor decided to treat the wound to prevent infection.

While assisting in splicing conveyor belt. employee was pulling belting into position. lost his grip. his right elbow struck against angle iron sideboard support causing a cut.
