EE was cleaning rock off EE's trailer with a broom after being loaded with 2-4" rock by a loader. EE was unable to reach all of the rock with the broom so EE climbed up on the trailer finish cleaning off. As EE was getting down, EE jumped the 3 1/2 feet to the ground and felt a pop in EE's lower leg, resulting in a trimalleolar fracture of the right ankle.

Showing all 7EE hooked a chain to one side of a plate and the other end was connected to the excavator. The operator lifted the boom to flip the plate, EE pushed the plate with EE's hands causing slack in the chain, which slipped off the plate and landed on EE's feet.
Employee was using shovel to clean out on side of plant, when employee misjudged shoveling and struck rt knee with shovel.
Operator was digging for boulders, front of excavator was slightly elevated, bucket slipped off of boulder, causing excavator to slam to ground. This caused to windshield to shatter. Glass shards cut operator on left wrist.
Person entered the mine site after hours, with the intent to commit a crime & fell to his death. The person ignored all signage, fencing and berms and entered the mine site.
Employe was cutting a conveyor belt, pulling the knife towards him, when it slipped and ran over the top of his 4 left fingers. This resulted in having stitches put on 3 of fingers, on the outward side of his left hand.
EMPLOYEE BENT OVER TO PICK UP A ROCK WEIGHING APPROX 20 LBS. UPON ATTEMPT TO LIFT THE ROCK TO MOVE IT, HIS BACK WENT OUT.
