While drilling with a hand drill, drill bit caught metal causing torque of drill. Drill handle spun around and made contact with right eye lid.

Showing all 23Conveyor belt wasn't turning on. Turned power off to check fuses. An unexpected electrical shock to left hand and ball of flames on chest and face. Second degree burns on left hand, chest, and face.
Standing on conveyor to repair chute, the conveyor was accidentally activated causing employee to fall and left leg to be twisted and pulled.
Fixing a 988B Cat loader, putting a new bucket pin in and it slipped out and smashed left thumb with hammer. Thumb needed sutures.
Service truck's rear end was lifted with out-riggers. EE was on driver side and co-worker on passenger side. As truck was lowered, they were watching for broken parts. EE put head between tire and service body of truck in order to see better. As truck was lowered, EE's head was pinched between tire and body. EE received multiple head fractures.
Rock was stuck in scalp screen. Used pry bar to loosen. Pry bar slipped and struck face causing laceration needing stitches and loosen teeth.
Employee was stepping out of the loader to add oil to tank. He had a hold of the handrail and was stepping down. His foot slipped, due to rain, and he strained his left shoulder.
The miner was walking along the crusher to access a breakdown. When he turned around he had stepped on unstable ground causing him to sprain his ankle.
Cleaning rock from jaw when a chip of flying rock struck the middle finger of left hand causing a cut.
EE was welding side rails on belt. Suspect piece of hot slag went under skirting at feeder hopper end. After welding crew was on site 40 add'l min. cleaning up & no sign of fire. Shift ended 6:45 pm. Slag evidently smoldered & started fire on skirting. EE usually runs belt but since was end of shift, he didn't. Damage consisted of burning up a belt & some rollers on conveyor.
He fell off conveyor and hit neck.
Was standing on rocks to perform tasks and after finishing stepped off rocks hyperextending his knee. On 7-28-08 he said he hurt his knee and thought he had done it at home.
He was putting a return roller back up under the over load conveyor, when he felt a sharp pain in his neck. Came to work on 4/22/08 and reported it and went to ER that day.
Original DOI was 11/09/08, when the grinding disc broke and smashed R. thumb. No treatment at that time. Over the next 3 months a bump appeared, so he sought treatment for a bone spur. He had surgery to remove it on 3/12/08 and returned to work on 3/13/08.
HE RAN INTO BOBCAT SKIDSTEER WHEN TURNING AROUND AND CHIPPED HIS TOOTH.
Checking water level in radiator on impact lost footing, struck front of face on railing.
WHILE TIGHTENING BOLTS ON LOWER PART OF JAW ROCK BEING FED IN HOPPER, ROCK FELL OUT & BOUNCED OFF PLATFORM & FELL HITTING HEAD & SHOULDER, THEN FELL TO KNEES.
WHILE ATTEMPTING TO REPOSITION A SKIRT RUBBER ON A CONVEYOR WITH A BAR, THE BAR CAUGHT BETWEEN THE BELT & A ROLLER CAUSING THE BAR TO STRIKE HIS KNEE.
WHILE OPERATING 988 FEED LOADER THE REAR WINDOW CAME LOOSE AND STRUCK OPERATOR ON BACK OF HEAD & NECK.
ROCK FELL FROM 10'-12' OF ELEVATION DOWN ON RIGHT FOOT STRIKING HEEL & INSTEP. ROCK BOUNCED OFF OF THE TOP OF THE SCREEN. EE WAS SHOVELING ROCK FROM UNDER THE CONVEYOR.
CABLE SLIPPED OUT OF HANDS, TRIED TO RE CATCH IT AND CAUGHT BURR ON RIGHT HAND, CAUSING 1 1/2" CUT.
CHAIN ON DRILL BROKE, DRILL FELL AND SMASHED FOOT AND PINNED FOOT.
SHOVELING OPPOSITE SIDE OF JAW WHEN A ROCK FELL OFF JAW & STRUCK TOP OF HELMET STRAINING NECK.
