Crusher Operator was lifting impact rails about 25lbs over his head to co-workers. After lifting the fifth rail the operator felt pain in his right shoulder that started to radiate down his right arm to fingers. The operator returned to next scheduled shift, after a few months shoulder did not heal operator was seen by doctor whom suggested surgery.

Drill Operator was pulling the pull rope to start a 5 HP generator. The operator felt discomfort in his back and reported it to his supervisor. The Drill Operator went to chiropractor for adjustment.
Crusher Operator was dismounting the frame of a Tertiary Crusher when his foot slipped and his other foot hung up on the arm guard of the cone causing his knee to hyper-extend.
On the nightshift of 7-31-2014 the Haul truck operator finished fueling his truck and accessed his haul truck ladder and claimed to hurt his back the HT Operator reported the incident 8-4-2014 AM. The HT operator was not released to return to work after Appt. on 8-7-2014.
Operator was walking on unconsolidated material when a rock shifted under his foot he reached down to balance himself and cut his right hand on a sharp rock requiring three stitches.
Craftsman was shoveling snow and lifted, twisted and turned simultaneously and caused pain to his lower back. Went to chiropractor for treatment.
Pad Operator 2 was operating the backhoe pulling pipe off of the leach pad and started to notice tightness in his right upper part of back. Operator sought treatment at chiropractor.
Operator was changing loader edges and was removing the edge bolts. One of the edge bolts fell through the grove and EE held it back up with his left hand and when he used the air wrench the bolt caught his glove and hand and lacerated his thumb pad the operator required stitches.
Supply Clerk unlocked the double doors to the warehouse filter storage building pushed the lever down, pulled the lever to open the door and the handle broke tearing through his glove and lacerating his right index finger.
Operator was descending a fixed ladder when left foot stuck in the ladder rung and twisted his knee as he reached with his right leg for the next rung. The operator suffered slight tendon tear and started physical therapy on his left knee 1-27-2014.
Production loader operator had his right foot sitting in an idle position while operating a loader with his ankle rolled to the side, it felt like it was starting to fall asleep. He proceeded to walk around the loader to relieve the pain and was unable to.
Eaton construction was using a guzzler truck to excavate material next to the Assay Lab. An EE was using the wand and handed it to IE and IE was unable to keep it from passing over his left foot. The high pressure cut a hole in his boot and foot about 3/8" in diameter. IE was unable to return to his next scheduled shift.
Plant Operator was adjusting a pipe manifold over the top a protective rubber mat with the help of a co-worker. When he and the co-worker lifted the pipe he felt a pop in his neck and shoulder. The operator did not feel medical aid was required at the time, the operator then sought medical aid on 8-30-2013 and 9-3-2013.
Employee was assisting a tag line when he hit a bracing bar on a stationary belt with his left knee and the knee hyper extended. He did not seek a doctor until 8-30-2013. The doctor has recommended physical therapy.
Drill Helper was attaching alligator clips to 12 volt battery to power a diesel fuel transfer pump, when he did so he felt a shock and pain to his right arm.
Exploration drill helper moving a pipe back to the rack stand. Helper did not use the proper method to handle the pipe twisted and turned instead of lifting and walking with the pipe. The helper has experienced severe back pain and will be evaluated medically before return to work.
Operator was tramming a track drill on a snow covered bench when he hit a loader tire rut approximately 8" deep. When the drill came out of the rut the front tracks went up into the air and slammed to the ground jarring the operator while he sat in his seat.
Employee was walking down traction type plastic on a slight grade and he felt his knee give out. It was determined to be a tear to ACL and Meniscus, worked until surgery on 12-6-2012.
Installing Rolling Steel door, putting curtain of door into guides and employee's finger got pinched between the curtain and the wall. This cut the tip of his middle finger on his right hand.
Operator was walking from his haul truck to the shop. The operator slipped on the ice and fell to the same level. The operator was assessed at the emergency room with a fractured left humorous bone.
Operator was cutting a piece of flat cardboard with a pocket knife, the knife did not cut all the way through in an area. The operator then pushed harder with the knife causing it to slip and cut his left hand at the base of his thumb. The operator was transported to the ER room where he received (2)two sutures.
Operator was changing GET using deadblow hammer and punch when a piece of the GET flew off and penetrated operator's left leg above the knee. The operator is scheduled to have the metal removed on 7-27-2011. The operator has not missed any work to date.
Employee was working on a blast pattern when he fell for no apparent reason. The supervisor recognized the signs of a stroke and quickly transported the employee to the mine first-aid station for further evaluation. The employee was later treated for a minor stroke at a hospital. We do not believe that the employee's work environment played a part in the onset of this illness.
Employee lifted two bundles of lathe and as he was doing that felt and heard a pop in his shoulder. The employee did not see improvement went to doctor on 5-3 and the doctor diagnosed surgery needed on 6-1-2011.
Haul truck was being loaded with spent material, there was some frozen chunks in the material. The loader dumped the bucket of material in the truck box and a large piece of ice hit the offside of the haul truck jarring the truck operator. When the operator was jarred he experienced back pain, the operator was recently released by a physician for a back injury unrelated to work.
Operators were winterizing an eight inch poly line. Air was blown through the pipe and was exiting a 2" valve on the other end. An operator ensured that all air had exited the 2" valve. An operator disconnected a 4" quick coupler and the air that was still in the pipe knocked the operator to the ground. The medical determination was a sprained MCL. The investigation is on going.
Mechanic was removing defuser from hydraulic filter with hydraulic filter still in the housing. Pulled the defuser out with pliers and the whole filter came out, pinching the middle finger between filter and housing. Mechanic received five sutures in middle finger.
Employee was sliding a large rock off of a dozer arm. When the rock began to fall he twisted his body to get out of the way. He experienced pain in his left side middle back.
No specific incident occurred. A small fragment of steel became inbedded in the employees eye while performing routine clean up at the crusher. Injury was determined to be work related by Workers Compensation on 07/06/2010.
Employee was digging a trench with a spade. Impact from the hard ground while digging caused pain in left shoulder.
Employee was adjusting a newly installed door in the crusher building. Adjustment was above head level causing employee to look up while adjusting. A gust of wind blew a foreign object into his eye.
Employee was walking along a stream bank when he slipped on moist vegetation. When he attempted to catch his fall with his hand, his hand contacted a tree branch, causing a laceration.
Employee was washing a 2 liter glass beaker when it broke, resulting in a laceration to right index finger that required 5 stiches
Employee was using an air impact wrench to loosen a bolt on a cutting edge. Bolt came loose sooner than expected. Torque of impact wrench made wrench rotate up striking the person on the lip. Laceration to lip required stitches.
Employee experienced pain in left arm due to repetetive motion. Employee was seen by a physician. No medical treatment received, but was put on restricted work activity.
Employee was carrying a framed picture to his vehicle, when the glass in the frame broke and fell on his hand. Laceration to left hand, required 10 stitches.
Sand truck operator was sanding the haul road into the Deep Portland pit. Approximately mid way down the ramp, the operator applied the brakes to slow down and they did not slow the truck down. All attempts to stop the truck failed. Operator steered the truck into a pile of material at the bottom of the ramp. Operator received 2 stitches on chin for cosmetic purposes.
Employee was stretching out a 3" water hose and handing the nozzle to another person. As the hose unwound, the nozzle rotated and hit the employee on the lip. Two stitches required to cloase laceration. Dental work will be required to repair chipped tooth.
Operator was moving drill parallel to a cutbank when the material on the crest gaveway. Drill tipped over on it's side and fell into trench. Operator was ejected out of the drill through the window and came to rest when he hit the drill mast. Operator received two broken ribs.
Employee was sitting in a Caterpillar 777F 100T haul truck getting loaded by a Caterpillar 992G loader. With the full bucket of the loader over the box of truck, the loader operator pushed the wrong lever causing the bucket to fall onto box of truck. Truck driver was tossed around in cab causing strained neck muscles and a sore thumb.
Employee was helping to lift up a hood on a straight truck when he planted his right foot on uneven ground causing his knee to pop and cave in. An MRI revealed a torn ACL and the employee was advised on 09/05/2007 that surgery was required to repair the ACL. A date for the surgery has not yet been scheduled.
Employee was using a vise grip, holding a punch to pound out a u-joint on a pick up truck driveshaft. Hammer glanced off of punch and hit left index finger. X-ray revealed a fractured bone.
Employee was ascending two steps to entrance of Warehouse office, when she experienced pain in the back of her right knee. Doctor visit revealed pulled hamstring. Employee unable to work next scheduled shift.
Employee was picking up the end of a tow strap when he felt a pull in his lower back followed by slight numbness in right leg. Doctor visit revealed a pulled muscle in lower back.
Employee was kicking in drill cuttings into a blast hole to get to designed hole depth when he felt something "pop" in his left knee. An MRI reveaqled torn meniscus and the employee was advised on 03/29/07 that orthoscopic surgery was needed. The surgery has been scheduled for 04/06/2007.
Employee was moving a portable fuel tank to a sump area. While in the process of connecting fuel lines his face struck against a piece of tin sticking up above the tank. Laceration to face requiring stitches.
Employee was putting oil in a Cat 16G Motorgrader. when he turned to walk on tandem housing. When he placed his foot on the tire he slipped and fell to the ground. Weather conditons were snowing. Slight oil spill may have contributed to the accident. Injury consists 3 fractured ribs.
Employee was cutting a piece of plastic liner with an exacto knife when his left hand. slipped causing the knife to come away from the plastic. Momentum of the hand and knife continued on cutting his right forearm. Laceration to right forearm.
Employee was loading haul trucks. loader tire went into a soft spot on floor, employee experienced pain in his lower back as a result.
