THE BUCKET OF THE FRONT END LOADER THE EE WAS OPERATING BECAME LODGED IN THE GRAVEL HE WAS EXCAVATING, CAUSING THE REAR WHEELS OF THE LOADER TO LIFT OFF THE GROUND. AS THE BUCKET WAS FREED, TH E REAR WHEELS CAME BACK ON THE GROUND CAUSING THE MACHINE TO ROCK. THE EE, WHO CLAIMED TO BE WEARING HIS SEAT BELT, FELL FORWARD AND STRUCK HIS HEAD ON THE WINDOW OF THE MACHINE.

EMPLOYEE WAS REBUILDING AN ENGINE. THE GEAR ASSEMBLY WAS ON A WORK BENCH & WHILE ROTATING THEM MANUALY WITH HIS LEFT HAND, HE CAUGHT THE TIP OF THE MIDDLE FINGER OF HIS RIGHT HAND IN THE GEARS .
