On 9/18/12, he left work around 3:00AM without notifying anyone of his injury. He called-in sick on 09/19/12. He came back and worked the following days; 9/20, 9/23, 9/25, 9/26, 9/27 and 9/28. He then called in 9/30/12 and 10/1/12.

EE was helping install safety chains on the presip catwalk. he was holding a latch for EE White to weld. The welding rod accidentally while showing another ee were he should hold the part, and touched the part. a spark came off and went over EE safety glasses into his left eye lid.

Hydraulic fluid got into the eyes of the forklift operator for the warehouse. After taking several moments to wash his eyes out with eye wash and water, he deemed it necessary to seek medical attention because of prolonged irritation.

Employee was climbing up # 52 elevator on cooling side of silos with an MSHA inspector. when he got to the top he felt his left butt cheek start to hurt. Turned out to be a pulled muscle.

Employee was removing masking tape from an air line in the bag house, when the air system was turned on. This employee's head was right next to an air vent when the system came on next to his left ear.

The subject was changing a brake chamber when a spring struck him on the lip.

The employee said that he may have hurt his knee getting off a forklift, but he wasn't for sure. There is pain and swelling around his knee.

The miner was inadvertently injured when his left shoulder struck the window latch in his designated haul truck.

Employee was moving steel in shop when something cut his finger (Right Hand). He thought it might be a sliver of metal, but not sure .

Employee was pulling out oil heaters when some kind of liquid got under glove and burned back of right hand. We believe the liquid may have been a degreaser.

Employee was shoveling pile of material and pulled something in his left shoulder.

Employee was changing bags at Kiln #2. He picked up a pallet and stepped on a dirt clod and twisted his left ankle.

Employee was working with hole punch and had left thumb in the way and another employee slammed hole punch down on left thumb of employee.

Employee was pumping oil into Kiln tank. Employee was sprayed a little with the oil causing a chemical burn to his left forearm on the inside.

Employee was cleaning under 5 pound line when a dust shield came off. He tried to put it back on and got his left hand caught in belt.

Employee was shoveling material and turned the wrong way causing his lower back the left side to strain and pull a muscle.

The employee was stacking 50 lb bags onto pallets, when he felt something pop in his left wrist area.

Employee was inspecting cylinder for air leak when the cylinder closed catching the employees right pinky finger in trap doors.

A conveyor was being moved as it was being moved. It the handle on an air valve to the on position.This caused the air line to whip around hitting the employee several times on his right shin causing cuts and bruises. The air valve in question is on kiln deck plant # 2

Unplugging crusher, when pry bar got caught in crusher wheels, bar then came up and hit EE under neck. Knocked EE out, chipped teeth, hit head and hurt back.

Employee was stacking 40# bags, when he caught one off the conveyor line, he spun around and felt pain in his left shoulder area.

Employee was getting on skytrack, when he pulled the left side of his lower back.

Employee was stacking 25# bags onto a pallet, when he said his lower back started to hurt. He then said it was starting to hurt worse so we put him on a stretcher and called 911.

EE was moving an empty tank from the ground to set it on top of a freightliner flat bed. The crane stopped. EE jumped off and slid on the ice/snow twisting his ankle.

Employee was removing empty pallet from stack to place in scissor lift. Top pallet was caught on another pallet and employee jerked pallet to get it loose. In doing that as the pallet became free his left hand was caught between both pallets.

Employee had hand over auger box and finger went through duct tape, employee thought it was metal cut his right index finger and had to have stitches.
