Employee may have been trying to perform maintenance on the tail pulley or conveyor belt, when employee became entangled in the conveyor system, causing fatal injuries. The operator is continuing to investigate the root cause(s) of this accident.

Contractor cutting on arm of excavator, grease fitting & grease shot out hitting contractor. Contusion to right shoulder requiring 2 stitches, 1st degree burn to left side of face & 2nd degree burn to left collar bone both requiring first aid treatment. Contractor states was released from hospital with no restrictions related to this accident.
Employee was waiting in line to be loaded. When it was his turn the loader operator signaled for him to pull up. When he did not pull forward, a co-worker went to check on him. He found employee slumped over in his seat.
Miners were removing a counterweight from an excavator, standing side-by-side, when the counter-weight dropped unexpectedly to the ground.The victim stumbled backwards,fell landed on the back of his head on the shop floor. He was unresponsive during CPR. The autopsy report concludes the proximate cause of death as"commotio cerebri"noting also cardiac arrest.
hydraulics failed on Volvo 360 trackhoe, causing bucket to come to the ground, jarring the cab. Caused muscle pull in employees back.

Employee was performing maintenance tasks when he twisted his right wrist while installing a gear box at the plant.
Employee was priming pump, the pump float moved and flipped. Employee jumped over railing that was around pump. He caught the back of his leg on the railing. He landed in the water. He was wearing a life jacket and pump was disconnected from the power and was locked out.
Pulled left calf muscle when climbing up onto trackhoe.
Mechanic/operator was working between two buildings. A window & frame was propped up against building. The window was sitting at angle and the operator walked into it and cut his leg on the frame corner.
Mechanic was working on Mack road truck adjusting clutch. He asked the other mechanic to depress clutch from inside the cab. The mechanic had not moved his hand when he asked the other mechanic to depress clutch and when the clutch was depressed the mechanic caught his middle finger on his right hand severing the top of his finger .
Employee was climbing into dump truck, his foot slipped on bottom step and he fell onto his back. Low back pain, hip pain and muscle strain.

Employee rec'd training on 5-11-07 practices for getting on and off equipment.
Employee was knocking slag off a piece of metal with a hammer, he slid his safety glasses down his nose because he said that they had fogged up and a piece of slag hit him in the eye.
Employee climbed onto hopper to direct trackhoe. Trackhoe bucket hit sideboard on top of hopper. Side board swung down. Another side board that was attached to sideboard that was hit swung forward hitting employee; employee fell forward and into hopper. He hit his arm on a six inch eye beam injuring left arm. Employee received a strain/contusion of the left bicep muscle.
Employee was tightening bolts on screen at plant when his tool slipped and his arm came back and he hit his left elbow on plant. Contusion to left elbow area.
Employee was sitting in buddy seat of the Case off road haul truck. He reached across his body with his right hand to close the door which was on his left. He shut the door on his left arm. Contusion to left elbow area.
EMPLOYEE WAS TURNING A BOLT WHEN PUTTING IN A PIN IN THE BUCKET OF THE VOLVO EC360 EXCAVATOR. THE WRENCH SLIPPED AND HE HIT HIMSELF IN THE FACE WITH THE WRENCH CAUSING A LACERATION TO THE LEFT SIDE OF HIS FACE.
Employee was picking up a 50-lb bag of chemical when he strained his back. He has a prior history of back problems that we were not aware of until this injury occurred.

EE WAS OPERATING A FRONT END LOADER AND SAID HE STRUCK A HOLE, SAID HE HIT HIS HEAD ON ROOF OF LOADER AND JARRED HIS BACK, SAID HE WAS WEARING SEAT BELT, SITE WAS INSPECTED BY SUPERVISOR. NO H OLE FOUND. DOCTOR IS STATING THAT THIS IS A QUESTIONABLE CLAIM AT THIS TIME.
EMPLOYEE WAS WALKING FROM GREASE SHED TO PLANT HOUSE. HE CROSSED THE HAUL ROAD AND WAS STRUCK BY THE BUCKET OF A MOVING LOADER. REFUSED MEDICAL CARE AND DRUG SCREEN.
WAS CARRYING PIPE WITH CO-WORKER. PIPE WAS DROPPED AND IT FLEW UP AND HIT EE IN THE MOUTH.
WAS USING A HAMMER (HAND HELD) TO KNOCK IN A PIN AND HIT HIS HAND INSTEAD OF THE PIN.
WAS SCRAPING HAUL ROAD WITH TRACTOR & BLADE. DROVE TRACTOR OFF SIDE OF ROAD. TURNED TRACTOR ONTO ITS SIDE.

EE WAS SHOVELING UNDER A GUARDED PULLEY. HE HIT THE UNDERSIDE OF THE PULLEY WITH THE SHOVEL AND THE SHOVEL SPRANG BACK FRACTURING HIS FINGER. EE WAS TERMINATED FOR FAILURE TO PRODUCE URINE SPE CIMEN.
EE SLIPPED ON SOME SLIPPERY STEPS EXITING THE PORTS TRAILER AT THE BIG PLANT. HE HAD A PREEXISTING BACK AILMENT PRIOR TO THE FALL THAT IMPAIREDHIS ABILITY TO WALK CONFIDENTLY THE FALL RESULTE D IN WORK LOSS & RESTRICTED DUTY. THE INJURIES AQUIRED FROM THE FALL WERE BACK SPASMS & CHRONICBACK PAIN
WAS CLIMBING OUT OF THE LOADER AND MISSED A STEP, FELL TO THE GROUND AND HIT HEAD.
EMPLOYEE WAS GETTING DOWN OFF OF DOZER AND HIS FOOT SLIPPED OFF THE STEP AND TO GROUND CAUSING INJURY TO KNEE.
