Employee was drilling a hole in a piece of pipe, using an electric drilll mounted on a stand. As he pulled the pipe out, the drill bit caught his glove and pulled his left hand index finger into the hole saw blade.

Showing all 6An employee in the mill was using a handle extension to release a chain binder. When the chain binder released, the pipe handle extension flew off the handle and struck his left hand breaking his middle finger.
As EE was using a grinding wheel to remove paint and rust from skip he turned his head to look ahead of his work, a particle flew in the side of his safety glasses and lodged in his eye. It did not bother him until the next morning. It was removed by his dr.
AS HE WAS STEPPING OUT OF CAB, HIS FOOT SLIPPED OFF THE STEP AND HE FELL TO THE GROUND, STRIKING HIS HEAD, RIGHT RIB CAGE, RIGHT ARM & ELBOW. AMBULANCE TOOK HIM TO EMERGENCY ROOM,STITCHES IN H EAD & FIRST AID ON RIBS.
EE WAS BARRING DOWN LOOSE ROCK IN THE RAISE. THE 1ST ROCK KNOCKED OFF HIS HARD HAT AND THE 2ND ROCK HIT HIM ON THE TOP OF HIS HEAD, RESULTING IN A CUT. HE WAS TREATED AT THE CLINIC, AND RETURN ED TO WORK THE NEXT DAY.
WHILE DRILLING IN THE STOPE ON 9 LEVEL A LOOSE ROCK FELL FROM THE BACK ONTO HIS RIGHT HAND HOLDING THE JACK LEG DRILL. FRACTURED BONE IN HIS RIGHT WRIST.
