Rock fell off screener striking employee on the head, causing a laceration to top of head. Employee was below screener using a torch to install a guard.
MECHANIC WAS DRILLING OUT A BROKEN BOLT ON LOADER BUCKET WHEN DRILL BIT BROKE AND A PIECE OF DRILL BIT FLEW UP INTO INJURED'S FACE. DRILL BIT FLEW UNDER SAFETY GLASSES AND CONTACTED LEFT EYE C AUSING IMMEDIATE PAIN FORCING EE TO SEEK MEDICALATTENTION. WE ARE STILL AWAITING OUTCOME OF SURGICAL REPAIR TO DETERMINE ANY FUTURE ISSUES OR POTENTIAL DISABILITIES OR LOSS OF USE.
3 EE WERE ATTEMPTING TO LOWER A LEG FROM THE WASH PLANT DOWNWARD ONTO LANDALL TRAILER. WHEN CENTER OF GROUTY SHIFTED 2 EE LOST GRIP AND DROPPED LEG. SIDE R HAND WAS PINCHED BETWEEN LOADER BUCK ET AND LEG OF WASH PLANT AS IT CAME DOWN TO THE TRAILER.
TWO EE'S INSTALLING A "BANG PLATE" ON END OF CONVEYOR WHEN ONE EE DROPPED HIS END OF PLATE. EE FINGERS (RIGHT RING & PINKY) WERE PINCHED UNDER PLATE RESULTING IN LACERATION INJURY.
TRYING TO REPLACE A GUARD THAT WAS LLOSE USING A PRYBAR. PRYBAR SLIPPED AND PINCHED FINGER. LACERATION TO LEFT RING FINGER. FRACTURE TO LEFT RING FINGER.
RIGHT INDEX FINGER INJURED WHILE TRYING TO PULL A SCREEN OFF OF A LOADER BUCKET. THE INJURY REQUIRED STITCHES AND WAS WRAPPED TO KEEP IT CLEAN. THE EMPLOYEE REPORTED FOR WORK AT THE START OF H IS NEXT SHIFT.
HE WAS CLIMBING DOWN OFF THE EXCAVATOR, MISSED THE LAST STEP, AND BROKE HIS ANKLE WHEN HIS FOOT HIT THE GROUND WRONG.
EMPLOYEE CLIMBED HAND RAIL OF THE CAT WALK OF THE CRUSHER OPERATION. HE WAS ATTEMPTING TO ADJUST THE BAFFLE PLATE FOR STONE DEFLECTION. A STONE CAME OUT AND STRUCK HIM ON FOREHEAD. HE FELL OFF RAIL ABOUT 10' TO GROUND.
