Employee came in and the loader did not want to start, he climbed in and out of the loader. When the loader started, he climbed out and as he was climbing in, he stepped on the first rung, and as his hand reached for the side ladder rail, his hand slipped out and he went backwards and placed his hand down to cushion the fall. He was wearing work gloves at the time.

While employee was pulling on a ratchet in our Centron Tunnel, the ratchet failed and spun causing his body to fall. This fall occurred on flat ground, however, he fell so hard that it caused a break in his right ankle. The employee was immediately rushed to the hospital where he was treated.
