EE & COWORKER WERE REPLACING SCREENS ON SHAKER DECK. THE COWORKER LET SCREEN PANEL SLIP FROM HIS HANDS. THE SCREEN PANEL FELL, CATCHING EE'S LEFT HAND BETWEEN THE SCREEN AND SHAKER FRAME.
AFTER TIGHTENING CONE CRUSHER, EE WAS ASKED TO MOVE AWAY. HE STEPPED DOWN ON BELT UNDER CRUSHER. CO-WORKER WAVED FOR HIM TO GET OFF BELT & CRUSHER OPERATOR MISTOOK COMMAND FOR HIM TO START EQUIPMENT. WHEN BELT STARTED UP, EE WAS FLIPPED OFF BELT TO GROUND.
WE WERE CLEANING UNDER A CONVEYOR. THE SHOVEL HIT A STONE AND MY LEFT KNEE TWISTED.
EE WAS PUTTING ON RUBBER SPLASHING WHEN BOX KNIFE SLIPPED AND CUT HIS RIGHT LEG. HAD TO HAVE 5 STITCHES.

EE WAS USING AIR WRENCH TO TIGHTEN TRACK PADS WHEN A PIECE OF SMALL ROCK FLEW IN RIGHT EYE.
