The ground person working at our wash plant, was walking past a conveyor and reported that EE felt something strike EE's eye, but that it didn't hurt and would have it looked at after work. Later that afternoon EE was found by an ER Doctor to have a rock chip behind EE's eyeball and required surgery to remove it. Unknown damage or impairment at this time.

Showing all 3Excavator operator was stepping out of his excavator and missed the handhold while climbing off of his machine. Operator has a smashed ACL in his knee and two broken ribs. His knee was injured when he missed the handhold and fell to the ground landing on his feet. His ribs were cracked when he stumbled forward and fell on the track of the machine.
Customer came into the mine site to buy a load of rock and had a heart attack.
