MDI Mining · Struck by flying object

Employee was operating Jaw Crusher mining machine when the jaw became plugged. The employee was attempting to unplug the jaw when a piece of rock/debris flew from the machine and struck the employee in hard hat, penetrating the PPE. The impact caused a laceration in the employee's forehead.

Loss of footing and slipped off ladder, striking robs on upper deck. Rib contusion on his left side.

MDI Mining · Struck against a moving object

Hauling 4000 gallons of water up steep grade, truck came out of gear. Operator, unable to get it back in gear, decided to back truck down the hill. Watching for traffic on the right, he negotiated a turn to the left going backward. He failed to see a high spot on the bank and the left drive tires went up on this spot causing the water to shift laying the truck on its side.

Injured was conversing with excavator operator about loads while standing on track. Injured then tried to step off track facing forward instead of using foot steps. He lost his footing and slid down track causing abrasions to his lumbar area. The grouser on the track pad causing the most abrasion and discomfort.
