EE was working to re-secure a guard after belts being place on drive puller. EE placed a ladder on side of plant. After placing the 60' ladder ee went up several rungs. EE was placing a bolt in a hole and lost stability. Resulting in ee slipping off ladder and getting foot caught in process

EE told worker to clean out a plant then left to load trucks out. When he returned he got a ladder out to climb up to check belt tension without notifying the plant operator. He never locked the plant out. Plant operator turned the belt on and employee was out of visual sight.
