EE was helping maintance change a screen on shaker #3. He was lifting the screen and lost his footing. When he slipped the screen fell on his left hand cutting left ring and middle finger.

Showing all 12Employee was taking the barge out to the dredge to fuel it. While tying off the wind blew the two together and he smashed his finger.
Employee was trying to unjam the hopper at sand plant by hitting it with a hammer. He did not see the rock on top. The vibration knocked the rock off and hit him in the head causing a concussion and a bruised spinal column.
Employee stopped her truck and got out to move a ten inch rock at dump site. While she was getting back in her truck she said her back popped.
Employee was cutting a torn piece off the primary conveyor belt. His knife slipped and cut the top of his left hand.
Employee was cleaning his loader bucket. A rock was wedged between loader bucket and cutting edge. Employee was using a bar to knock it out. When the rock popped out the cutting edge popped back knocking the bar back against the bucket with his hand between bar and bucket.
Employee was standing on belt frame to grease feeder bearings and did not lock out belt. The belt was bumped causing employee to slip damaging his left foot or shin and spraining his ankle.
