TRUCK WAS STOPPED FOR REFUELING, DRIVER DID NOT SET PARK BRAKE. TRUCK STARTED TO ROLL. OPERATOR REMOUNTED TRUCK THEN JUMPED FROM BUMPER TO GROUND SPRAINING HIS LEFT ANKLE. TRUCK ROLLED TO BERM AND STOPPED.

EXCAVATOR WAS WORKING ON A SOLID LIMESTONE BENCH, AT A SLIGHT SLOPE. DUE TO FROZEN DONDITIONS IT SLID BACKWARDS AND LAID OVER ON RIGHT SIDE. OPERATOR INJURED RIGHT SHOULDER.
