Showing all 8Mechanic was working on a Bell articulated truck engine, removing misc exhaust hoses. He was leaning on a wrench and slipped, causing his forearm to come in contact with something that resulted in a laceration. The right forearm was affected.
Water pump motor had stoped running. He thought it needed to be primed before he started it back. The discharge line was full of water up a step grade. The water had sit it pump and gotten hot. He removed prime plug and hot water sprayed on his left hand burning it.
Hitting ratchet with 3 lb hammer, it kicked back hitting his thumb and bending it back out of the socket.
Employee got under shaker, removed guard from tail pulley and placed it leaning on shaker frame. He turned around and the vibration of shaker made guard fall on his back. On 8-26-05 employee was told not to be around shaker on tail pulley when it was running. This was the reason he was starting work at 5:30 a.m.
EMPLOYEE WAS LIFTING MATERIAL BY HAND AND STRAINED SHOULDER.
EMPLOYEE WAS REPLACING A FLYWHEEL. THE FLY WHEELSLIPPED OUT OF THE HOUSING FALLING ON TOP OF FINGER THAT RESULTED IN IMPACT WITH THE FLOOR.
EMPLOYEE WAS REPAIRING A SMALL CONVEYOR BELT DURING A RAIN. EMPLOYEE LOST FOOTING DURING REPAIR AND FELL APPROXIMATELY 5 TO 10 FEET TO THE GROUND.
