EE was helping his co-worker changing out a return roller on the PC2 conveyor. Upon completion of the task, co-worker handed EE a 3lb. hammer. EE stated that he started to "drop" the hammer so he swung the hammer upward to keep it from falling and during this action the hammer handle struck him on the mouth chipping his two front teeth.

Showing all 31Injury occurred while the employee was stepping up a small, weedy embankment after assisting his supervisor in repairing a flexible discharge hose on a 3" pump. Employee stated that he felt pain in his left calf muscle area and had difficulty putting his body weight on that leg.
One employee stopped and "locked out" the belt. EE went up the catwalk to inspect the head section. EE noticed a trough idler out of place & proceeded to put it back into it's framework. The other EE had finished his inspection took off his lock and "bumped" the conveyor. When the conveyor started EE's hand rolled between the trough idler and belt.
Employee was stacking cribbing and experienced low back pain.
Employee was doing "utility" work,(shoveling), in the C6 tunnel. While shoveling he felt a "pull & pain" in the right side of his abdomen & groin area. EE, went to see a doctor on 05/25/2010 and called off work until 05/31/2010. ee returned to work on 06/01/2010. There were no "restrictions" noted about his "return to work",.
Mechanic, was climbing the access ladder of a CAT 992C loader to check a reported problem. Upon climbing the access ladder he felt a "pull & burning" in his abdomen. He reported the incident to supervisor the next morning. EE saw his doctor at a later date who said he would need to schedule a "hernia" surgery.
Belt clamps were being used with "lever style chain hoist" to pull the belt together. During this process the internal brake of that lever hoist slipped which shifted the pressure to the lever hoist that employee was operating. The sudden shift caused his right little & ring fingers to get caught between the chain of the lever hoist and a trough idler frame.
Employee, Stripping & Mobile Equipment Supervisor, arrived at the upper,(Wymps Gap Road), entrance. When he exited his vehicle to open the gate he lost his footing on some ice & snow, causing him to fall backwards onto his back & right side.
Bottom deck sub frame sections were being removed from under a inclined screen. Injury was on the work platform(downhill side)when a section that was having to be "pried down" started a 2' "free slide" after coming into a "loose" fit area, He slipped while moving away from the section & fell backward onto the same work platform. *No safety chain was utilized.
While operating a haul truck, EE was attempting to locate a "rattling" noise which was coming from the rear discharge vent of the AC unit located inside of the passenger jump seat. EE inadvertently inserted his fingers into the vent while "feeling" around for what was making the noise. EE had 3 fingers contact the blower, which cut the fingers and lost nail of his right hand.
2 plant EEs were washing out from under the 'outbound' scale. A piece of walkway grating had been stood up against the scale house for access to the scale. After wash was completed the IE was pulling the water hose out of the access area by pulling the hose while walking backwards. He then tripped on the concrete curb, fell & cut earlobe on grating.
Injury had exited the scale house to retreive a file from a company vehicle. While walking back towards the scale house, Injury was stepping up from the scale apron to the walkway, approx. 8" height,& was not sure if she slipped causing her to fall forward landing on her knees,hands, striking her head on the metal walkway & rail.
Injured worker (employed by ABZ Transport LLC, Contractor ID# A706), reported that he slipped on some ice striking his right arm and hurting his back. He then got back into his personal vehicle and went home. Weather conditions at the T.O.A. were high winds, cold temps., sleet / rain combination.
While making wet hole blend off of pump truck employee felt pop and burning sensation in shoulder.
While driving a 777B CAT haul truck the employee hit his head on the cab ceiling going over rough section of the travelway. The operator was using this machine while his normal truck was being serviced. He was wearing his hard hat at the time of the accident. He later told his supervisor about the incident but said he was not going to seek any medical treatment.
The employees were shimming the jaw crusher to close the discharge setting. Two steel wedges were used to assist the hydraulic pump in opening the push block to receive more shims. While re- leasing the push block after the shimming was added the two wedges were removed the second wedge flew upward while being loosened with hammer striking EE in the upper lip
The primary plant operator and a co-worker were trying to avoid on-coming fly rock from and overburden shot by entering into the primary MCC #2 building. The injured individual was struck by a piece of fly rock that came thru the south east side of the builing. Blasting services were being conducted y Mt. State Bit Service, Inc. MSHA ID #C4R.
Employee was doing clean-up work with a water hose when he stepped off of the concrete foundation into a soft spot. The employee lost his balance but his right leg was stuck and his right knee was strained while he was trying to catch himself.
Standing in mud, using a wash out hose. EE went to move his feet - upper body moved, but right lower leg & foot did not, causing a "popping" in knee. EE did not report this until supervisor noticed him limping 4 days later. EE did not get med. attn. until 3 days following acc. EE has been working during physician's eval. - finally decided surgery required to replace meniscus.
two men install bottom rollers on excavator frame. He use an 3/4" air impact wrench with a 6" extension,swivel attachment to snug roller bolts. He hold air wrench in an inverted position from kneeling position making socket at about height. While tightening one of the bolts the socket slipped off bolt head then came off the swivel attachment and struck him in the face and mouth.
The dozer operator assigned to level a section of haul road left the area without re-establishing a berm for dumping. A haul truck entered the dump area and upon backing up to dump allowed the right rear wheels of his unit to start over the edge of the fill causing the truck to roll over on its top. The operator who was seat-belted managed to remove himself and call for help.
Ee was unplugging a chute using a long post hole digging bar. While working with the bar the ee pinched his right hand between the bar and the chute wall.
In the process of opening the jaw, (removing the shims), the swing jaw did not open. Ee was looking for the obstruction having the heel of his shoes behind the toggle push block that now had approx 5" of free travel space. Swing jaw suddenly released which forcibly pulled ee's feet backward into the solid back plate of jaw crusher, causing them to slap into the wall.
EE WAS WALKING ACROSS THE PRIMARY PLANT EQUIPMENT LOT TOWARDS HIS PERSONAL VEHICLE AFTER HIS SUPERVISOR OTHER TOLD HIS CREW TO SHUT DOWN C=BECAUSE OF HEAVY SNOWFALLS WHICH WARRANTED CLOSING TH E FACILITY. EE WAS WALKING ON A FLAT LEVEL SURFACE WHEN HE ENCOUNTERED ICE CAUSING HIM TO FALL. EE REPORTED TO THE ON SITE EMT THAT HE HEARD A CRACK/POP SOUND DURING HIS FALL. HE WENT TO THE
EE WAS STARTING HIS SHIFT FROM THE EMPLOYEE PARKING AREA. UPON THE START OF WALKING FROM HIS PERSONAL VEHICLE TOWARDS A HAUL TRUCK, EE WAS MAKING A TURN WHILE MAKING A STEP. HE APPARENTLY STEP PED ONTO A FIST SIZE ROCK AND IMMEDIATELY HURT HIS KNEE CAUSING HIM TO FALL TO THE GROUND.
EE WAS REMOVING AN ACETYLENE & OXYGEN BOTTLE FROM A SERVICE TRUCK FOR USE AT A CLOSER LOCATION. AFTER RELEASING THE MOUNTING BRACKET, EE PROCEDED IN REMOVING THE ACETYLENE TANK. WHILE HOLDING THE TOP OF THE ACETYLENE TANK ON THE GROUND THE OXYGEN CYLINDER, (CAPPED), FELL TOWARD EE APPROX 4 FT PINNING THE PALM OF HIS HAND BETWEEN THE PROTECTIVE RING OF THE ACETYLENE CYLINDER & THE O
EE WAS SHOVELING OFF A STALLED CONVEYOR. WHILE USING THE SHOVEL TO CLEAR OFF THE CONVEYOR, EE SAID HE FELT SOMETHING "POP"IN HIS GROIN AREA. EE DID NOT NOTIFY ANYONE OF THE INJURY. ON 9/27/ 03, EE WENT TO THE HOSPITAL BECAUSE OF INCREASED PAIN AND OBVIOUS PHYSICAL CONCERNS. HE IS SCHEDULED FOR A HERNIA OPERATION.
EE WAS WORKING WITH A CREW DOING A CONVEYOR BELT REPLACEMENT. EE SAID THAT AT THE TIME OF THE INJURY HE SIMPLY WAS MOVING HIS RIGHT ARM WHEN HE "FELT SOMETHING POP" IN HIS SHOULDER AREA. THE M EN HAD BEEN WORKING WITH CUM-A-LONGS PREVIOUSLY. EE TOLD A CO-WORKER & THE PIT FORMEAN THAT HE HURT HIS SHOULDER BUT DID NOT GO TO THE DOCTOR UNTIL 8/26/03.
WHILE WAITING FOR THE MAIN HAUL ROAD TO BE PREPARED FOR HAULING DOWN THE MOUNTAIN, INJURED DECIDED TO FUEL HIS HAUL TRUCK AT THE FUEL STAGING AREA ON THE TOP SITE. WHILE WALKING AROUND THE TRU CK TOWARDS THE FUEL DISPENSOR, HE BEGAN TO SLIP ON ICE IN A SPIRALLING MOTION ALLOWING HIS RIGHT LEG TO GET UNDER HIM AS HE FELL TO THE GROUND, BREAKING HIS TIBULA & FIBULA.
EE WAS WORKING WITH A CO WORKER DISLODGING A PLUGGED UP CHUTE AT THE SECONDARY PLANT. WHILE MOVING THE JAMMED MATERIAL 12"X0" HE LOST HIS BALANCE WHEN THE PLUGGED MATERIAL SHIFTED. HE WAS CAUG HT BY A CO WORKER DURING THE FALL BUT CONTACTED THE CHUTE WALL INT HE RIB CAGE AREA. HE WAS CHECKED BY A DR THE FOLLWING DAY AND HAD BRUISING ONLY. ALL EE WERE RE INSTRUCTRD AS THE FALL PROTE
GROUND CONDITIONS WERE WET FROM EARLIER RAINS. EE WAS RE-MOUNTING THE HAUL UNTIL USING 3 PTS OF CONTROL RULE WHEN HIS LEFT FOOT SLIPPED WHILE STEPPING WITH THIS RIGHT FOOT RESULTING INT HE EE CATCHING HIMSELF WITH BOTH ARMS. NO IMMDIATE INDICATING OF A PROBLEM PHYSICALLY UNTIL APPROX 30 MIN. LATER WHEN DISCOMFORT INCREASE. EE ASKED TO LEAVE AND GET IT CHECK WITH NO ASSISTANCE NEEDE
