Showing all 11Employee was splitting stone on the splitter, standing too close. A stone was ejected out of the mechanism and fell on top of employee's foot above the safety toe portion.
Rock fell on finger
06/08 EE was picking up a piece of Dry-stack stone and felt sharp pain lower back. EE worked on 06/09. EE rested over weekend and felt good enough to go back to work on 06/12. Pain in lower back worsened and EE went to chiropractor on 06/15. EE worked on 06/16 and rested over the weekend. EE worked on 06/19 for a full day and filed a report of injury and went to clinic on 06/19.
EE came in the office Friday afternoon, stated while lifting a rock footing was unsteady & caused a jarring in shoulder as EE slipped. EE stated EE has had previous surgeries to knee & shoulder, so EE knows when EE is hurt. Finished work early after completing 3 pallets of rock. EE was going to rest the shoulder over the weekend. Was stacking rock, my feet slid out from under me, the rock crushed my hand.
EE was stacking ledge. There was a roll of fencing wire on the ground and EE stepped on it and the end was exposed.
Smashed middle finger on right hand between rocks.
