Showing all 6EE was sitting on frame of crusher tightening bottom deck screen bolts, when wrench slipped and EE fell approx 3' and landed on the conveyor blet guard.
EMPLOYEE WAS IN BETWEEN TWO CONVEYOR BELTS. HE WAS GUIDING SERVICE TRUCK BACK TO DO A REPAIR. TRUCK BUMPED CONVEYOR AND IT SLID INTO HIM, PINCHING HIS LEG AGAINST ANOTHER CONVEYOR.
Employee got something in his eye. He reported wearing safety glasses. Said he may have had something in his eye for some time. Does not recall getting hit in the eye with anything.
Apparently he stepped off crusher and bumped knee on a steel beam.
WIND HOWLING AND DUST GOT IN HIS EYE. OPERATOR WAS IN FRONT END LOADER AT TIME OF ACCIDENT.
