Showing all 6Miner was operating bulldozer went over a rise on the terrain. As EE came down on the other side, the dozer shift up causing EE to strike the roof due to not wearing seatbelt. Sprained neck taking to emergency room for first aid.
Poor weather conditions caused the ground material to become slick which resulted in the employee falling.
Operator drove the excavator into a sump hole. He was in an area that he should not have been. The operator was trapped inside the cab for approxomately 1.5 hours. The operator was extracted and had no injuries.

CURRENT WORK CONDITIONS HAVE AGGRAVATED PRIOR BACK INJURY. OPERATIN BACKHOE AT SHEPPARDS WHERE THERE IS A HARD LAYER OF SHELL ROCK OVER DIRT HE IS DIGGING.
