The employee was cleaning the chute to the crusher when a boulder rolled over on EE's right foot. EE's foot was bruised and swelled.

EE was stepping down from the screen plant and sprained ankle.

While bagging 50lb bags of rock dust the employee heard a pop in elbow. This was only reported to us on 06/06/2016 at 2:30pm by the employee.

Trying to move a part strained back muscle.

The employee was installing a v belt on a Cedar Rapids crusher. He got the tip end of his ring finger between the shiv and the v belt and the crushed rolled and caught his finger breaking it.

employee was lifting a wheelbarrow full of rock. employee felt pain in the groin area.

Employee was bagging rock dust and became weak feeling and dizzy. Employee was brought into break room. He was complaining of numbness in right arm and weakness in his legs. Ambulance was called and arrived to pick up the employee around 12:00pm

Employee called in from home and stated that this condition had been bothering him for years. Employee thought he had a pulled a groin. Sent him from home to doctor. He chose to go to the emergency room. Attending Physician examined and diagnosed a hernia. Physician recommended that he see his personal doctor.

Slipped and fell on hydracone platform. He was not wearing fall protection. There were handrails around the platform. He was only 2-3 feet from platform.

EMPLOYEE PULLING BEARING OFF A SHAFT, PULLER BROKE AND HIT HIS RIGHT KNEE CAUSING SWELLING.

HE WAS CHANGING SCREENS AND FELT SOMETHING HURTING IN HIS BACK AND SHOULDER.

He was changing a pulley, grabbed frame and pried bearing loose; hand slipped and got mashed between frames, mashing right thumb.

EE WAS GRINDING AND SPARKS CAUGHT HIS CLOTHES ON FIRE. HE TRIED TO GET HIS SHIRT OFF AND BURNED HIS SIDE AND HANDS.

EE WAS HAMMERING ON A PART AND PIECE OF STEEL HIT HIM NEAR HIS LEFT EYE.
