APPLYING NEW RAILS ON THE CATWALK FOR HAUL TRUCK, RAIL SLIPPED AND FELL CAUSING LACERATION TO LEFT HAND
Employee was replacing a broken belt standing in a pile of sand. He swung around to cut the other side of the belt, his left foot was stuck in the sand. He sprained his left ankle.
EE was moving and replacing the impeller shoes. EE felt a pain in his lower back. EE was sent to doctor who restricted him for 7 days, but he has not return yet.
After refilling water can, employee felt pain in lower back from lifting water can.
EMPLOYEE REPORTED PAIN IN RIGHT ARM ON 12-7-05 WHICH OCCURRED ON 12-5-05 ROLLING A ROCK OFF OF DUMP HOPPER.
EE WAS PUSHING GREASE DOWN IN SERVICE UNIT. PLUNGER ON GREASE PUMP CLIPPED A LITTLE PIECE OF SKIN OFF HIS INDEX FINGER ON RIGHT HAND.
EMPLOYEE WAS REMOVING A ROCK FROM BETWEEN DUALS ON UNIT 90 WHEN ONE TIRE BLEW OUT. HE REACTED BY TURNING AWAY WHICH RESULTED IN HIS TWISTING HIS LEFT KNEE.
REMOVING TIRE ON SCRUBBER, TIRE SLIPPED PINNING FINGER BETWEEN RIM & HUB, BRUISING MIDDLE FINGERON RIGHT HAND.
