Pinched left hand ring finger from belt skiving tool.
Probable foreign material in left hand. Treated w/ first aid until 1/23/24, when explored wound site and found nothing. Wound site closed. Reporting out of an abundance of caution.
While climbing ladder to excavator, employee's foot slipped, resulting in discomfort in right calf. Injury did not require medical initially. Injury became reportable on 11/8/23 when employee reported swelling and bruising and injury became lost time.
Scalp laceration and back pain from fall off skid steer.

Reporting out of an abundance of caution. Employee reports a sore shoulder Monday morning. Reports it started hurting over the weekend, away from work. Reports no incident at work but feels it could have resulted from work. ILC questions causality.
While doing maintenance with a pry bar and chain fall, the chain fall hook broke resulting in the pry bar slipping, striking the employee's left arm causing pain and discomfort.
Employee states hurt back while pulling an air hose to pump up a tire at the shop. Occ med assessment is a lower back strain. Reporting out of an abundance of caution. Validity of injury on work site unsure.
Employee stood up and struck EE's head on a vertical bracing just below EE's hard hat resulting in a laceration between EE's eyebrows
Miner attempted to step on a short ladder leading to a conveyor service platform. Left foot slipped on ladder rung and right shin and knee hit the service platform mezzanine causing trauma to the knee cap. Miner did not have left foot in full contact with the ladder rung, only the tip of EE's boot. Ladder missing hand extension on the West side so did not have 3pt. contact.
Contusion of right knee when pry bar slipped and struck knee.
Employee claims discomfort in arms & shoulders while shoveling/sweeping. Employee was new hire and claims injury 1st hour of 1st day doing physical work. Pre-existing conditions are confirmed by doctor. WC fraud suspected and question if this is even MSHA reportable.
While draining a leaking hydraulic hose, oil sprayed onto employees face, entering employees right eye causing irritation.
Hatch on tanker trailer had bounced closed. Employee climbed up trailer to open hatch w/o fall protection and fell to ground. Employee incurred broken jaw and ribs, brain hemorrhage & contusions to liver, kidney & lung.
While cleaning material with a walk behind skid steer, employee's left hand became pinched between the hand controls and a beam. Index finger was cut requiring 2 stitches.
While unplugging the cracker mill, operators hand slipped and became caught in the tensioner assembly, resulting in a laceration of the ring finger, right hand.
Employee claims they fell to the ground when pushed by a co-worker, suffering a lumbar strain.

Condition was a hot humid day, no air movement in the area of cleaning. Drank water but not enough and didn't take multiple breaks during activities of housecleaning. At the 9am break didn't eat anything other than drinking water because they left their lunch pail at home.
While on knees, employee reached under an auger and reports feeling a twinge in back.
While operating a multi-position ladder at bulk load out, employee pinched left hand between ladder rung and ladder keeper.
While stepping off a ramp onto a trailer top to work on a hatch, employee reports feeling left knee buckle.
Plant was replacing an auger that was suspended with rigging straps. The motor and drive were attached, offset and balancing at the 12:00 position. The motor & drive unexpectedly became unbalanced and rolled to the 6:00 position, pinching the employees right hand. Cuts to the index and middle fingers required stitches. Improper rigging allowed the motor and drive to roll.
The miner was repositioning a 50lb bag to be re-inked and twisted back resulting in a muscle spasm.
Employee using a hand grinder and got a piece of metal in right eye
Setting a beam in place and it shifted and caught hand.
Line of Fire. Individual was in the shop cutting a conveyor belt with a utility knife. Was in a sitting position and knife slip and punctured left lower leg - calf.
The employee took off bearing cover of a screw conveyor when hot grease sprayed his forearm, hand and face.
EE had talked to another EE between 10:00 & 10:30 and told other EE that the plant was running ok. At approximately Midnight-12:05 other EE found EE on the shop floor with his eyes open and apparently not breathing. Other EE ran to the scale office and told another EE to call 911 that he had checked for a pulse and could not find one.
employee was shoveling material into a pile and aggravated the ulnar nerve in the left elbow.
Employee was taking a two piece cover off of an elevator door with a chisel and a hammer to pry the two pieces apart. The chisel became dislodged from between the two pieces and struck the employee in the forehead above the right eyebrow.
Back pain. Operating Quarry haul truck over haul road. Operator indicated being jarred but could not identify actual area of occurrence. Haul road in fair shape other than small rutting due to rain and stripping activities, but not severe.
We had a light snow / ice event that occurred early that morning that made the black top in the area slippery. He was on his way to punch in slipped on the black top and hurt his right leg. At this time doctor is indicating a pulled hamstring in the upper thigh area.
Employee climbed on top of a motor guard to check cleanliness of a Hammer Mill feeder pan. After checking, employee proceeded to climb down off of the guard and in doing so slipped and fell onto his back, striking either the guard or the concrete motor pad. Employee sustained fracture to the 8th rib on right side and sprain right ankle.
Cleaning belt with scraper when scraper was caught and pinched hands into side guard & tightening screw breaking right index finger degloving left index finger and bruising left shoulder.
EE was walking on graveled area, slipped and reached for a oxy-acent torch cart to catch himself and struck his forearm on the hose holder, causing a cut to his right forearm.
Was knocking dust from a fan and stuck his finger through the guard and cut his finger. The cut needed stitches. He will return to work when the stitches are removed.
HE WAS WORKING ON A DUST COLLECTOR WHEN HE SLIPPED OFF A LADDER AND TWISTED HIS LEFT ANKLE. HE WENT TO THE DOCTOR AND DR. SAID IT WAS A SLIGHT SPRAIN. REST IT FOR 24 TO 48 HOURS AND HE CAN COME BACK TO WORK WITH NO RESTRICTIONS.
HE WAS DUMPING A DUMP CART FULL OF MATERIAL AND PINCHED A NERVE IN HIS NECK. HE IS BEING TREATEDMEDICALLY WITH SHOTS AND THE POSSIBILITY OF SURGERY AT A LATER DATE.
