DRIVER WAS MAKING A 180 DEGREE TURN DOWN A HILL WITH A LOAD OF WET SAND. THE TRUCK DID NOT MAKE TURN AND WENT UP THE BERM & ROLLED TO ITS LEFT SIDE. DRIVER WAS UNHARMED. CONDITIONS CONTRIBUTIN G: HEAVY LOAD, SHARP TURN, STEEP INCLINE, INEXPERIENCE, & HARD STEERING.

EMPLOYEE WAS LIFTING A HEAVY PIPE OFF OF A SHEET OF STEEL HE NEEDED FOR FABRICATING. HE LEFTED THIS PIPE IN A MANNER WHICH CAUSED THIS INJURY.

INJURED WAS STEPPING DOWN OFF THE TRUCK WHENF RIGHT FOOT HUNG UP ON WIRE ROPE STEP, THEN LOSING BALANCE AND FALLING BACKWARDS. HITTING THE GROUDON HIS RIGHT SIDE AND ARM.
