Showing all 7Using power washer. Wand on power washer got away from him. Sprayed hot water on his left leg.
Coming out of excavator, slipped and hurt his knee. He did not report it till 12:45 PM. Another EE stated he stumbled on cable and fell and twisted his knee.
Working on sand atacker, employee fell approximately 24' into sand dome and injured his left arm.
Normal working conditions. While hammering block bearing on shaft, foreign object entered right hand & thumb.
Normal working conditions. Subject tightening bolts on crusher, wrench slipped and hit him in chest.
There are four 1/2 inch bolts that secure the coupling on the hose. Employee took one of the bolts off and as he was taking off the second bolt the line coupling separated and caused hydraulic fluid to spray into his left eye causing medical treatment at once.
