Showing all 5While climbing onto the loader, my foot slipped on the second step causing me to loose my hand-hold and fall approx. 4-5 feet to the ground. This caused a strain to my back and a sprain to my wrist.
While performing service work on dragline, a co-worker dropped a steel pin weighing approx 10lbs. The pin hit the deck and fell against his knee.
INDEPENDENT DRIVER WAS HAULING IN WAST-WASH OUT TO DUMP IN OUR PIT. HE LIFTED BED TO DUMP, PARTIAL LOAD GOT STUCK IN TOP OF TRAILER CAUSING WEIGHT OF UPLIFTED BED TO BE UNBALANCED AND ROLL TRAILER AND TRACTOR ONTO SIDE. DRIVER HAD SEAT BELT ON AND SUSTAINED SMALL CUT TO LEFT SIDE OF HEAD AND DISLOCATED SHOULDER
