Showing all 6Employee was repairing the hydraulic hose on a Caterpillar D6N Dozer. He was climbing over blade arm to tighten leaking hose and slipped. he fell against tracs and pulled shoulder back.
The employee was stripping at gravel pit. The employee had dumped his load of dirt and was returning to the load site. At this time he chose to travel over a pile of dirt. as he was traveling over the pile of dirt he got too close to the edge of the pile and the equipment overturned and landed on him.
PERSON WAS TAKING UP BELT WITH ADJUSTMENT BOLT. BOLT BROKE AND WRENCH HIT EMPLOYEE IN HEAD.
EE WAS USING A MANAUAL GREASE GUN TO GREASE DOZER. HE SLIPPED OFF DOZER. HIS LEG SLID BETWEEN THE UNDER CARRIAGE AND BLADE ARM.
EE HAD COMPLETED CHECK OF OVERSIZE CHUTE AND WAS WALKING DOWN STEPS. SLIPPED ON ICE AND FELL. SPRAINED RIGHT ANKLE.
