Checked oil and water on 980c loader. Walked around counter weight while climbing left rear loader tire to put oil in transmission slipped on frost, tried to catch myself went to the ground about 5 feet and landed on my feet. Left knee, leg and left shoulder pain.

While cleaning up and handling heavy metal screens, anvils and slingers, during the process of the day, the EE's shoulder became painful with crunching and popping sounds. Accident did not become reportatble until 5/24/07, when EE sought medical attention.
Working on water pump, employee and I were holding up the pump, while I turned the pump house. EE's foot was on the floor of pump house and twisted his leg while I was turning pump house and hurt his back at the same time.
Employee was caught in shaker box
Swinging a 20 lb sledge hammer, hammer was deflected and hit right foot.
WHILE LIFTING ONE HALF OF A 4" CRUSHER SCREEN, I FELT A PULLING SENSATION IN MY LOWER BACK, FOLLOWED IMMEDIATELY BY A VERY INTENSE PAIN.
