EE was getting tools off the trailer. EE went to walk around the trailer, tripped, landed on arm. EE went to doctor to just get checked out. They said EE inflamed EE's rotator cuff tendon.

EMPLOYEE WAS ATTEMPTING TO LOOSEN A BOLT, PUSHING THE WRENCH AWAY FROM BODY WHEN THE WRENCH SLIPPED OFF THE BOLT AND THE TOP OF RIGHT INDEX FINGER STRUCK ANOTHER BOLT PROTRUDING OUT OF THE FACE PLATE RESULTING IN A LACERATION.
Operator pulled up to office at the end of shift; failed to engage parking brake. EE stepped out of truck to unlock hubs on 4wheel drive; the truck stayed still until EE unlocked the hubs then began to roll as EE walked around in front of it. EE turned; leaned into truck; as EE grabbed hold of it their wrist became caught between the office & grill of truck and broke it.
Two men putting a part in place, when one of them let go of his side leaving the other with the full weight of it. Causing the first man to twist his back and pull a muscle.
Body found floating in lake by Dredge Operator 5/6/2008 @ approx 9:45 am. The body had been in the lake long enough to surface, therefore, the date and time of the accident is unknown. No positive identification was made.
Truck parked on incline and rolled over right foot.
Employee was filling the steam cleaner with Gunk Liquid Pressure Washing Compound. As he was filling the machine, the can slipped and some of the fluid got on his face. He was taken to Smith Northview Hospital to be seen by a doctor as a precaution. He was seen by the doctor and released back to full duty. There was no burns or other visible injuries.
Miner was loosening bolts on the dredge pipeline to add another piece of pipe. As he was trying to loosen the bolts the wrench slipped causing his hand to strike the pipe. When his hand hit the pipe it tore the fingernail off of his right index finger. He was taken to the doctor to have it looked at.
Using a hammer and a cable cutter to cut a piece of cable for the dredge. A piece of the cable chipped off and hit the employee in the eye. He was wearing safety glasses at the time of the injury.
Suction pipe on dredge stopped up at 9:00 a.m. Two miners unbolted suction pipe to remove roots. When bolting suction pipe back on pump, employee stuck his right index finger in the bolt hole to check alignment; pipe shifted and the tip of his finger was cut off. He received 3 stitches and returned to work the same day.
EE WAS WORKING ON DREDGE. WHILE TRYING TO INSERTA PIN HE WAS HOLDING ONTO AN EXCAVATOR BUCKET. HIS RT. FOREARM RUBBED ACROSS EXCAVATOR BUCKET WHICH HAD A STEEL BURR ON IT WHICH CUT HIS RT. FOREARM.
