An employee was opening the inspection door to a rock crusher when the door slipped out of his hand and hit his left forearm. This resulted in a contusion of the left forearm. He will be limited to only using his right arm for what appears to be a few days.

The employee was removing a tray of #5 stone from the Gilson screener when he felt a sharp pain in his lower left back and leg.

The associate was cutting a piece of rubber for a rock shaker when the knife that he was using slipped and cut the back of his left hand. The cut was approximately 1/2". It does not appear that any damage was done beyond a soft tissue flesh wound.

Employee was adjusting the outrigger on his mechanic's truck. Held on to outrigger unit by placing his finger in the lowest adjustment setting hole. His hand slipped and the outrigger foot fell trapping his finger between the outrigger and the outrigger housing.

Employee was changing flights on the coarse material washers. He stepped from the top of the tool box to the bed of the repair truck and twisted his ankle.

Employee was attaching grease lines on haul truck, Dirt fell from the grease line and struck his eye. Employee was wearing safety glasses.

Employee states the he was pulling on hose to clean up Steadman crusher and injured his left shoulder. This was reported to the supervisor around 9:00am on 9/25/12, company was notified that employee had MRI which indicted a torn rotator cuff.

Employee was dismounting haul truck, foot got caught causing him to fall. He tried to catch his fall with arm and hurt arm. Went for X-ray on 5/8/12, results indicate wrist is broken.

Employee was repairing an electric motor and hoisting it when his right pinky finger got caught between a cable and the motor. He had to get six stitches.

Employee was helping change a screen, claims he hurt shoulder on 1/19/12. Employee states he told supervisor but supervisor does not recall being told. Employee stated on 2/2/12 that shoulder was hurting and wanted to seek medical attention. Originally there was no lost time but subsequently had surgery on 3/29/12.

Employee states he fell while getting off of a loader. He alleges he mis-stepped near the last step of the loader & fell to the ground. He tried to catch himself with his arm & fell on it as well. He originally thought the fall was not serious. He had continued pain and saw a doctor that put him on modified dutyuntil Oct 5th. The Dr. susequently took EE off of work.

Employee was coming down from having cleaned material from a conveyor belt. When he stepped down, he stepped awkwardly on a block and fell to the ground approximately three or four feet.

Employee was driving a maintenance truck down the quarry road when he lost control of the truck & it flipped. He was trapped in the vehicle until emergency response personnel retrieved him. He received stitches in his ear & was bruised. He was seen at the ER & discharged on same day. He was instructed to not return to work until results of next checkup on 5-13-09.

Employee states he was trying to lift a motor from a shelf and that he hurt his lower back in doing so. Employee originally continued working but has since experienced continued pain. Employee is presently scheduled to have back surgery in May and is not working due to an enlarged disc.

While repairing an engine, employee was trying to remove a part with a pry bar when it slipped causing him to cut the top of his left thumb.

Employee originally stepped awkwardly in a dip in the ground on 1/8/08 hurting his right knee. Employee continued to work. He then states that he re-injured the same knee on 2/16/09. He continued to work. His knee has evidently worsened and he had knee surgery on 3/27/09. Employee has since returned to work, seated duty only, on 4/15/09.

Employee was stepping off the loader. As he stepped down from loader, as his right foot touched the ground, he felt a burning in his left leg. Employee tore muscle in left calf.

Employee was operating a haul truck. He alleges that the seat collapsed. He alleges that his back was injured by the alleged seat collapse. He took a week's vacation the next week and resigned from the Compay on July 6, 2006.

EE was helping change a rock crusher hammer with another EE. During the process, the EE got his finger caught under the hammer causing it to be pinched and therefore cut causing 6 stiches to be received.

EE alleges that he was shocked while leaning against electrical box. Witness saw employee fall but cause of fall is still under investigation.

While lifting pipe with hands, the pipe slipped and a burr on the pipe cut his finger.

WHILE CUTTING A CONVEYOR BELT EMPLOYEE ACCIDENTALLY CUT HIS RIGHT THUMB WITH A BELT KNIFE

EMPLOYEE HAD PUT OIL IN EXCAVATOR AND AS HE GOT TO THE CATWALK, HE LOST HIS BALANCE AND SLIPPED ON THE CATWOLK, FALLING TO THE GROUND

Employee crawled through an opening to clean the bottom deck of a screen. While attempting to crawl out of the screen, he struck his right knee on the bottom of the screen housing

Employee was lifting a brush pile and felt a pain in his back.

Employee was assisting in the changing of a screen. a piece of dirt or rust apparently fell off of the screen wire as it was being installed and entered the ee's left eye.

EMPLOYEE WAS GETTING READY TO LEAVE WORK. HE STOOD UP IN BULLDOZER CAB AND ACCIDENTALLY HIT HIS HEAD ON CB RADIO. HE CUT HIS HEAD REQUIRING FIVE STITCHES TO FOREHEAD.

EE WAS BENDING DOWN TO TURN OFF WATER VALVE. THE WET SCREEN WAS ABOVE HIM & HE HIT HIS HEAD ON PIPE. EE HAD TO GET A FEW STITCHES.

EMPLOYEE WAS WORKING ON CRANE ENGINE WHEN HIS RIGHT HAND GOT CAUGHT IN THE FAN BLADE CUTTING THE TOP OF HIS RIGHT HAND.

EMPLOYEE WAS OPERATING BULLDOZER WHEN THE GROUND GAVE WAY CAUSING BULLDOZER TO ROLL ON IT'S SIDE CAUSING CUTS AND ABRASIONS TO EMPLOYEE (FIVE STITCHES TO THE HEAD).

EE WAS REPAIRING A LIEBHERR 974 AND WAS EVIDENTLY STRUCK BY LIGHTNING. EE WAS HOLDING A METAL HYDRAULIC HOSE WHEN HE HEARD A LOUD BANG AND WAS KNOCKED TO THE GROUND. EE HAS NO KNOWN INJURIES, BUT IS BEING HELD AT THE HOSPITAL FOR OBSERVATION.

EE STATED THAT ON 5/7/02 HE NOTICED HIS BACK STARTED TO HURT WHILE HE WAS WORKING. HE SAID NO SPECIFIC INCIDENT COULD BE NOTED THAT CAUSED THE PAIN TO START, IT JUST STARTED HURTING & GRADUALL Y GOT WORSE. HE DIDN'T INFORM EMPLOYER OR ANYONEELSE. WORKED THRU FRI, WAS SCHEDULED TO WORK SAT & SUN BUT DIDN'T. WENT TO DR SUN. 5/12, WHO SAID IT WAS A PULLED MUSCLE & PRESCRIBED MEDS.

EE CLAIMS HE HURT HIS BACK WHILE WORKING. EE STATED THAT ON THE NIGHT OF 4/16/02 HIS BACK STARTED GETTING SORE. THEN HE STATED ON 4/17/02 HIS BACK WAS HURTING SO BAD THAT HE SAID HE NEEDED TO GO TO THE HOSPITAL. HE WENT TO DOCTOR 4/18/02. HE TOLD THE DOCTOR THAT HE HURT HIS BACK KICKING A PIPE.

EMPLOYEE WAS TIGHTENING A BATTERY CABLE ON A LOADER WHEN HIS RING FINGER ARCHED CAUSING HIS FINGER TO GET BURNED.

EE WAS HELPING WELDERS WELD A SHAKER SCREEN. EE ASSISTED ALL DAY AND THAT NIGHT HIS EYES BEGAN HURTING. EE WENT TO THE EMERGENCY ROOM THAT NIGHT. DR BANDAGED HIS EYES FOR FLASHBURNS.

WHILE WASHING UNDERNEATH DRY SCREEN, A ROCK COME OFF OF THE CONVEYOR BELT AND STRUCK EE ON RIGHT ARM ABOVE WRIST CAUSING A LACERATION.

EE WAS PULLING ON A WELDING LEAD AND TRIPPED AND FELL ON HIS LEFT HAND RESULTING IN A BROKEN LEFT HAND,

A SEVERE THUNDERSTORM AND TORNADO WARNINGS CAM ABOUT ABRUPTLY IN OUR VICINITY. THE EE ATTEMPTED TO FLEE FOR PROPER COVERAGE & PROTECTION FORM THE STORM. THE EE WAS IN A TRUCK W/2 OTHER EES WHE N A STRONG SURGE OF WIND BLEWTHEIR VEHICLE ABRUPTLY. EES DECIDED TO GET OUT OF THE VEHICLE AND RUN FOR SOME OTHER TYPE OF COVER. AN ADDITIONAL STRONG WIND CAME ALONG & PICKED THE EES UP AND C

EE CUT FINGER ON LEFT HAND WHILE CUTTING PLASTIC PIPE.

WHILE GRINDING ON A PIECE OF ALL-THREAD ROD, EE LACERATED AND BROKE THE TIP OF HIS LEFT INDEX FINGER WHEN THE ROD BECAME HUNG IN THE GRINDER.

EMPLOYEE WAS LOWERING STEEL PLATE TO THE GROUND WITH CHAIN. HIS RIGHT HAND WAS STEADYING STEEL WHEN THE CHAIN SLIPPED OVER HIS FINGER. HIS FINGER WAS CAUGHT BETWEEN THE STEEL PLATE & LIFTING CHAIN CAUSING A CUT WHICH REQUIRED SEVERAL STITCHES.

EMPLOYEE WAS WORKING NEAR A FREE FACE. THE ROCK GAVE WAY OR SLIPPED UNDER HIS FOOT, CAUSING HIM TO FALL APPROXIMATELY 47 FEET OFF OF THE HIGHWALL.

EE WAS CUTTING A HOSE WITH A KNIFE. EE WAS PULLING KNIFE TOWARD HIMSELF AND WHEN THE HOSE CUT THE KNIFE INADVERTANTLY CUT THE TOP OF HIS LEFT ARM CAUSING HIM TO GET SEVERAL STITCHES.

EE WAS USING A KNIFE TO ENLARGE A BOLT HOLE IN A WEAR LINER. THE KNIFE SLIPPED CAUSING A STAB WOUND IN THE LEFT HAND.
