He was unplugging the jaw conveyor about 5 p.m. and lost his balance and fell off. It did stated at one time that he jumped off. When he hit the ground he twisted his ankle. It did not hurt until the next morning, May 13, and was not reported until then.

A CUSTOMERS TRUCK STALLED & EE JUMPED OUT OF THE LOADER & RAN UNDER THE CRUSHER TO GET A CHAIN TO PULL THE TRUCK OUT OF THE WAY. HE RAISED UP TOO SOON UNDER THE CRUSHER & HIT HIS HEAD & NECK , KNOCKING HIM FLAT. HE RECEIVED A 3/4" CUT ON THE TOP/BACK OF HIS HEAD.

EE BENT DOWN TO PICK UP SOMETHING AND HIS HARD HAT FELL OFF. BEFORE HE GOT IT BACK ON, HE WAS HIT ON THE LEFT SIDE OF HIS HEAD WITH A SMALL ROCK. HE HAD A HEADACHE, TOOK SOME ASPIRIN & RETURNE D TO WORK. HE WENT TO THE DOCTOR THE NEXT MORNING & RETURNED TO WORK.
