Employee was unloading a load of crusher run into the storage pile and failed to fully lower the bed of the A40 Volvo truck. He drove under the belt of the rock crusher and hung it with the bed of his truck and destroyed it. It was pulled from the building and bent and is no longer operational.

Employee was clearing a jammed rock from the jaws of the rock crusher with a long metal tool. The jaw caught the tool and thrust it upward and hit him in the throat/neck area.

Employee was dispersing a cluster of rock on the conveyor belt with a tamping bar. The tamping bar caught on the belt and was flung back and hit him in the chest.

The injured employee was helping to clear material from a jam in the crusher. Co-worker (uninjured) pulled a large rock from the crusher, causing him to stumble backwards; he miss-handed the rock to the victim and the force of the rock stripped the skin and tissue off the side of his left hand and little finger.
